Work History

Band 8a Senior Pharmacist Elderly Care

County Durham and Darlington NHS Foundation Trust
Durham, Durham
06.2023 - Current

As an independent prescriber, I have taken on a more specialist clinical focus, particularly within Parkinson's Disease where I am now a member of the trust's MDT, which involves reviewing patients in clinics and at home. It also requires me to be involved in developing shared care agreements for drugs such as apomorphine, and organizing the supply and delivery of the new levodopa subcutaneous prodrug Produodopa after recent NICE approval. I am also involved in audits surrounding correct timings of PD medication administration, and have previously completed an extensive audit around appropriateness of oral iron prescribing that has changed practice in the trust. I have also expanded on my leadership and educational responsibilities, line managing new junior pharmacists, and taking on the role of pregregistration/trainee pharmacist tutor. This is alongside all of my responsibilities mentioned in previous roles.

- Ward management roles - liaising the MDT to ensure seamless patient flow and evidence-based clinical care of patients

- Conducting clinical audits within elderly care regarding suitability of oral iron prescriptions based on recent BSG guidance

- Communicating with my numerous ward teams (hospital-wide) to solve stock and supply issues based on ward demand

- Ensuring commitment to self-directed learning and specialization through completing my clinical diploma in January 2023 and starting my independent prescribing course at the same time.

My roles as a junior rotational pharmacist included:

- Providing medicines and medicines-related advice or medicines information queries to both healthcare professionals and patients based on appropriate resources and evidence

- Ensuring appropriate and timely discharges and liaising with community teams for seamless transfer

- Therapeutic drug monitoring and dose adjustments during inpatient stays

- Being involved in MDT meetings such as ward huddles, COVID-19 MDT discussions for allocation of treatment, and RCA meetings

- Education of staff members including trainee pharmacists, junior doctors, and overseas OSPAP pharmacy students

- Overseeing the dispensary as the responsible pharmacist and clinically checking prescriptions before release

- Undertaking ward audits such as insulin audits to assess current practice and find areas requiring improvement

- Providing on-call/out of hours/late night/weekend services

- Ensure safe and secure handling of medicines on wards and within the pharmacy
