As an ambitious and hardworking nurse, my dedication has earned me success in becoming a CHC Nurse Assessor and Best Interest Assessor. Experience conducting DST assessments and leading on MDT’s, Checklists, Reviews, Fast Tracks, Fast Track Reviews, Funding Reviews, Complex Case Management, Duty, Needs Portray Documents, Retrospective Reviews and QAC. I strive for challenges and will go the extra mile to complete a task. I am an initiative-taker who works with minimal supervision if not none but still value the importance of teamwork. I have worked and contributed valuably as a member of multi-disciplinary team on Continuing Health Care assessments and throughout my nursing career. I have a wealth of experience from different Clinical Commissioning Groups working as a Continuing HealthCare Assessor implementing the National Service Framework for NHS continuing Healthcare and NHS Funded Nursing Care. Working as a CHC Assessor and Best Interest Assessor has enhanced my caseload management skills to meet NHS England guidance and key performance indicators for assessment completion. Working for the CCGs has improved my knowledge in analysing complex information provided from various sources and using different reporting systems. I have comprehensive and detailed assessment skills within a variety of settings, including care homes and domiciliary involving liaison with carers, other health professionals including GP’s consultations, hospital staff, specialist nurses, voluntary and statutory organisations and other relevant agencies. My current role has provided me with the importance of developing and reviewing sustainable and robust care management for less complex, unique, and individualised care packages within a variety of settings. I always strive for excellence in everything that I do and am enthusiastic about learning and development. I am proactive with initiating my own learning opportunities as well learning opportunities to benefit the team, colleagues or patients. I have a good understanding of self-awareness which allows me to reflect from mistakes and learning from them as well as working alongside others to identify ways to avoid mistakes in the future as well understanding my limitations and seeking assistance when the need arise. As part of my professional development, I actively seek feedback from colleagues, patients and managers about my work and performance and I value as well as learn from what they say. I set realistic and achievable targets to review on a regular basis. I am very keen to undertake appropriate education and support opportunities to develop my own practice. As an interim experienced RGN CHC Assessor I have worked with numerous NHS ICBs to help clear a backlog of live continuing healthcare cases. Performing high quality holistic assessments of current cases to enable the application of the National Framework and to identify the level of care needs.

BIA Best Interest Assessor/DoLS Assessor
As a dedicated and experienced Best Interest Assessor (BIA) in the Deprivation of Liberty Safeguards (DoLS) sector , I am committed to ensuring that care and treatment decisions for adults who lack the capacity to make decisions for themselves are made in their best interest. My role has been instrumented in making complex decisions and assessments, focusing on the principles of autonomy and dignity for those under my care.
My work is characterised by a strong track record of effectively completing Court of Protection Deprivation of Liberty (COPDOL) applications. This requires a deep understanding of the legal framework around mental capacity and deprivation of liberty, as well as a high level of attention to detail. Through this experience, I have honed my ability to navigate complex legal and ethical challenges, consistently advocating for the rights and best interests of those unable to do so for themselves.
In addition to my BIA work, I have successfully dealt with objecting cases, an aspect of my role that necessitates robust negotiation and mediation skills. I am adept at managing situations where there is disagreement over the best course of action, always striving to achieve the most beneficial outcome for the individual involved.
