CHC Nurse Assessor currently contracted to assist the ICB to meet its KPI targets and to reduce the CHC/ FNC/ Fast track review backlog. I complete three DSTs per week both virtually and on a face to face basis.

Contract Real Staffing

CHC Nurse Assessor employed on an interim basis to support the ICB with backlogs of reviews and also to help locality teams to complete DSTs in a timely manner to meet KPI’s.

I supported the ICB with review of patients receiving NHS funding within Individualised Patient Activity/Complex Cases - Mental Health, Learning Disabilities and Joint Funded caseload (Non CHC funding stream) and CHC Funded patients, on a face to face and virtual basis on Microsoft Teams.

• Completed accurate and comprehensive reports assessing and analysing placements and funding streams.
• Provided support to patients, families and providers.
• Liaised with our Adult Social Care colleagues and built a good relationship with the team.
• Completed caseload in a timely and efficient manner.
• Competent in using Microsoft Teams

As part of the CHC team embedded within the ICB, I was responsible for providing clinical expertise , reviewing both previously assessed and unasssessed periods of care.

I also:

• Led on the review of CHC disputes and CHC retrospective review requests to ensure that the NHS Continuing Healthcare processes have been adhered to.
• Produced comprehensive reports identifying a patients journey throughout the period of review.and presented my recommendations to Local Resolution Panel.
• Have an excellent understanding of NHS Continuing Healthcare and I have enhanced communication skills to chair Local Resolution meetings with claimants and their representatives.
• Provided support to families and their representatives to improve their experience of the NHS Continuing Healthcare process
• Managed my own caseload and I have assisted the ICB to reduce their backlog of Appeals and PUPOCs cases.

• Experience in all areas of Continuing Healthcare Appeals and Retrospective Cases including Local Resolution Meetings and dispute management

• CHC Nurse Assessor with experience in commissioning care , case management , and CHC/ FNC reviews in a busy CHC team completing 3/4 DSTs over 4 days.
• Excellent communicator with patients and their families, members of multidisciplinary team and legal representatives.

• Trained CHC Assessor and Discharge Co ordinator within the Acute Trust organising safe transfers ,placements and complex discharge planning.
• Completed CHC assessments and DSTs for both MLCSU ( Fylde and Wyre CCG ) and Blackpool CCG.
