Working as a staff nurse within the East Sussex school health service. The school health team works to promote and protect the well being of children and young people age 4 - 19 attending state school or being home schooled. My duties included:

• Comprehensive health needs assessments for all referrals
• Developing IHC plans for students in collaboration with parents and schools.
• Supporting both students and parents in promoting healthier lifestyles for students by means of packages of care (at tier 1) (including: sleep, physical activity, healthy eating, self care, transitions, emotional health and well-being, pubert,y sex and relationships).
• Signposting students and parents to relevant community providers.
• Safeguarding
• Following up on health needs identified by the school screening service.

Providing patients in the community with a comprehensive range of clinical nursing interventions including:

• Patient assessment and care planning
• Health promotion and education
• Wound care and chronic disease management
• Medication management and administration
• Patient advocacy and collaboration with the MDT
