My role involved creating and reviewing person centred care plans, ensuring each individual received safe and effective support tailored to their needs.

I worked closely with care staff, clients, families and healthcare professionals to maintain high standards of care and good communication. I managed staff rotas to ensure consistent and reliable care delivery while also supporting carers with guidance and problem solving.

I responded to emergencies and ensured all care documentation was accurate and up to date in line with company policies and regulations. Although this was a coordination role i remained hands on when needed and supported care delivery to ensure clients wellbeing and comfort at all times.
