I have 3 years working as a Care Coordinator where i was responsible for organising and managing care services for a range of clients with different needs. My role involved creating and reviewing person centred care plans, ensuring each individual received safe and effective support tailored to their needs.

I worked closely with care staff, clients, families and healthcare professionals to maintain high standards of care and good communication. I managed staff rotas to ensure consistent and reliable care delivery while also supporting carers with guidance and problem solving.

I responded to emergencies and ensured all care documentation was accurate and up to date in line with company policies and regulations. Although this was a coordination role i remained hands on when needed and supported care delivery to ensure clients wellbeing and comfort at all times.

Delivered consistent, high quality care to elderly clients over a 6 year period, building trusted relationships with clients and their families while delivering reliable and compassionate care.

Key responsibilities:

• Assisted clients with personal care including bathing, dressing and toileting.
• Supported elderly individuals with mobility, using safe manual handling.
• Administered and prompted medication in line with care plans.
• Prepared meals and assisted with feeding when required.
• Monitored clients health and wellbeing reporting any concerns to line manager.
• Supported clients with conditions such as dementia.
• Maintained accurate care records and followed all safeguarding procedures.
• Travelled between clients homes ensuring punctual and reliable care delivery.
