• Supporting the Registered Manager and Care Co-ordinators, ECM data checks, staff compliance and service user complaince, personal centered care plans, reviews, auditing service user journals, MDT meeting with social workers, district nurses and families

• I have gone from carer to co-ordinator in the company, in my co-ordination role i manage a team ofcarers which involves weekly rota's so service users have continuity with there care, implementing person centred care plans and risk assessments with the services users and their families, on call phone4 times a week, weekend cover, staff compliance
• I also have involvement with district nurses, doctors and social workers often attending MDT meetings
• I work closely with the registered manager and have in the past attended registered manager meeting
