Summary

I am a community-based trainee pharmacist with experience in hospital and GP sectors who is due to sit the June 2026 GPhC exam.

I believe that I am proactive and can contribute to a number of services offered within community pharmacies and provide patient centred care when doing so; including pharmacy first consultations, the new medicines service, contraception services over the counter advice. I have strong clinical knowledge and judgement and always intervene when I believe is appropriate. I am currently studying for my prescribing qualification and am specialising in ENT, of which I hope to develop in and continue to practice within a community setting. I am able to manage high workloads and communicate effectively within a multi-disciplinary team, ensuring that patient safety and care is always prioritised.

  • I have completed audits to ensure the pharmacy's patients who are asthmatic were on optimal therapies regarding their inhalation devices. Ensuring that medicines were optimised for these patients to enhance patient safety.
  • I have been able to accuracy check prescriptions and identify common dispensing errors in order to alter the dispensary layout and put in measures to prevent these errors recurring.
  • Throughout my role as a trainee pharmacist, I would say that I have confidently developed a strong professional, yet down to earth and friendly communication style amongst colleagues and also patients. By gaining experience in leading consultations for Pharmacy first and contraception services and the new medicines service, I have built a communication approach that has allowed me to build strong rapport with patients to allow them to feel open and comfortable in consultations.
  • I believe that I have developed strong clinical knowledge and judgement. I have been able to make interventions when I have identified drug interactions (such as with warfarin with naproxen or lithium with ramipril) or identified drugs or doses unsuitable for patients based on their comorbidities or drug risks (such as intervening when doses of escitalopram above 10mg have been given to over 65s or when cephalosporins have been prescribed for patients with severe penicillin allergies). I am competent and confident in providing pharmacy first services for all 7 ailments, involving history taking and identifying red flags that would require referrals to ensure patient safety is not compromised. I am confident in providing the contraception service (including emergency contraception) and counselling these patients on the risks of these medications and appropriate actions to take (involving the use of barrier methods for patients taking GLP-1 agonists). I am able to confidently lead NMS consultations and believe that I am providing patient centred advice, such as changing the timings of taking medications if certain side effects occur and counselling patients on when to seek medical attention due to the risks of their medications, such as bleeding risks with DOACs. I have specialised in ENT for my prescribing area and feel confident in safe prescribing for UTIs and ear infections that would normally be excluded under PGD. Liam Young-AR-260529-37091 I am also able to acknowledge my competency regarding prescribing and have made appropriate referrals in times where I did not feel comfortable or competent prescribing certain medications, such as oral itraconazole, for patients to ensure that their safety is not compromised.
  • As I have developed in my role, I believe I have gained more managerial skills and have been able to take a leadership role in delegating tasks. I have been able to adapt well to stressful high workloads and prioritise tasks, such as checking prescriptions and dosette boxes, providing services, answering OTC queries, dispensing installment scripts and organising orders to enable work to be delegated appropriately amongst staff - ensuring tasks are completed correctly to ensure patient safety is not compromised.
  • I have worked in a variety of sectors within my training year, such as two different community pharmacies that use different PMR systems (Analyst and Titan), as well as in GP and hospital setting that use Nervecentre. I believe that I have been able to adapt well to using these PMR systems, demonstrating flexibility and efficiency in obtaining computing skills and gaining familiarity with how different pharmacy settings operate. I would be very confident that I would be able to effectively adapt to the systems in place in any future workplace, especially if I were in a role that meant that I would be required to operate a variety of PMR systems.
