Summary

I am currently a Trainee Pharmacist. I am expected to obtain full registration with the GPhC in June/July 2026. I am currently completing my 52-week training placement within a hospital, including a nine-month rotation in an acute mental health inpatient setting with Leeds and York Partnership NHS Foundation Trust, during which I am on track to achieve my prescribing qualification in Psychiatry. Additionally, I am rotating through Leeds General Infirmary to broaden my knowledge of physical health management. This is a fixed term contract expected to terminate July 31st 2026.

I am particularly interested in applying these skills within a primary care setting, with a focus on managing long-term conditions and addressing the needs of high-risk patient demographics. I am motivated by the opportunity to reduce non-elective hospital admissions through proactive care, careful medication management and regular review of patients’ clinical risk factors, including age and medication regimes.

My hospital experience so far has emphasised to me, the value of a proactive, patient-centred approach in primary care and I am eager to transfer and further develop these skills to make a notable impact on patient outcomes within these communities.

I am a highly motivated and enthusiastic individual, particularly with regards to pharmacy practice. I exhibit a natural curiosity and a strong commitment to continuous learning, enabling me to further develop my skills and advance my professional growth throughout my career, particularly at this pivotal stage in the development of my professional and clinical knowledge.

  • Attendance to daily multi-disciplinary team (MDT) meetings to coordinate continuity of care with regards to inpatients and assess clinical suitability of current medication regimes and the social/holistic aspects of patients care.
  • Medicines reconciliations/Medication Histories- upon inpatient arrival and/or discharge to ensure medication regimes and allergy status are up to date, ensuring patient safety and optimisation.

  • Providing patients with relevant information, such as PIL and other mental health accommodating information leaflets etc.
  • Audit trails- to review SOPs relevant to pharmacy practice which highlight areas for improvement. Currently completing an audit relevant for highlighting prescribing improvement practices.
  • Ward support wherever appropriate.
  • Liaising with MDT HCPs to provide integrated and continuity of care and ensure completion of communications and/or documentation.
  • Completion of Prescribing hours in my nominated prescribing area of Psychiatry.
  • CPD-attending relevant training classes where appropriate to continuously improve on my learning. E.g. Morph Consultancy.
  • Worked effectively within a local (pharmacy) and wider (MDT) team to ensure smooth operation of patient care.
