

Motivated and patient-focused Trainee Pharmacist with experience working across community pharmacy and general practice settings. During my training year, I gained valuable experience within GP practices, undertaking medication reviews, participating in medicines optimisation activities, reviewing discharge information, supporting clinical audits, and working alongside multidisciplinary healthcare teams including GPs, prescribing nurses, and paramedics.
I have developed strong clinical, communication, and problem-solving skills through direct patient interactions and collaborative working within primary care. I am committed to delivering safe, effective, and patient-centred care while continuously developing my clinical knowledge and professional practice.
During my training year, I gained extensive experience working within a GP practice, where I managed my own clinics under supervision and conducted structured medication reviews for patients with a range of acute and long-term conditions. I reviewed medicines to optimise treatment, improve adherence, identify prescribing issues, and support shared decision-making with patients.
I was responsible for reviewing and actioning hospital discharge letters, completing medicines reconciliation, updating patient records, and implementing medication changes safely and accurately. I also completed a variety of clinical and administrative tasks, including responding to medication queries, supporting prescribing processes, and contributing to medicines optimisation initiatives.
To broaden my understanding of primary care, I shadowed GPs, nurses, and other members of the multidisciplinary team, gaining insight into patient assessment, diagnosis, and the management of a wide range of conditions. I regularly attended multidisciplinary team meetings, where clinical cases were discussed, patient interactions were reviewed, and learning was shared to improve patient outcomes and clinical practice.
These experiences strengthened my clinical knowledge, consultation skills, communication abilities, and understanding of collaborative working within a primary care setting, preparing me for a pharmacist role in general practice.
As part of my trainee pharmacist year, I worked across The Hollies Medical Centre, Rustlings Road Surgery, Nethergreen Surgery, and Carterknowle & Dore Medical Practice, gaining experience within a range of primary care settings.
During this time, I was involved in medication reviews, medicine optimisation activities, and clinical audits, developing my understanding of safe and effective prescribing. I also gained exposure to identifying patients who may be suitable for cost-effective medication changes and observed how prescribing decisions are made within general practice.
In addition, I worked closely with and shadowed prescribing nurses, paramedics, GPs, and other members of the multidisciplinary team. This allowed me to observe patient consultations, learn about the management of acute and long-term conditions, and gain insight into the delivery of patient-centred care within primary care. These experiences strengthened my clinical knowledge, communication skills, and understanding of the pharmacist's role in general practice.
