As an FY1 doctor, I played a crucial role in ensuring the smooth day-to-day running of the ward, taking responsibility for implementing consultant-led management plans and ensuring patient care remained efficient and safe. My responsibilities included coordinating referrals to other specialties, performing basic ward-based procedures such as venepuncture, catheterisation, and ABG sampling, updating the handover list with accurate patient information and plans, and scribing during ward rounds. Often, I was the first point of contact for nursing staff, requiring me to make rapid clinical assessments, initiate management where appropriate, and escalate concerns when necessary. In addition, I maintained a high standard of professional development, consistently updating my ePortfolio with reflections, workplace-based assessments, and evidence of competencies.
Throughout my FY1 year, I developed strong time management and organisational skills, learning to prioritise tasks effectively in a fast-paced clinical environment. I became increasingly confident in prescribing safely, adhering to guidelines while balancing patient-specific factors. A significant part of my growth involved learning when to escalate concerns, ensuring that deteriorating patients received timely senior input. As an FY2 doctor, I work on the middle-grade rota, alongside Internal Medicine Trainees, GP Trainees, and Specialty Registrars, allowing me to gain valuable experience in managing a wide range of acute and chronic medical conditions. My role involves assessing, admitting, and reviewing patients across multiple settings, including direct referrals from A&E and GP practices, as well as providing ongoing ward-based care. I am also responsible for supporting junior doctors, ensuring appropriate escalation, and facilitating safe and effective patient management.
This position has given me extensive exposure to diverse clinical environments, from outpatient clinics to high-intensity night shifts in the medical high-dependency unit (HDU). I have had the opportunity to work within multidisciplinary teams, contributing to ward rounds, liaising with specialist teams, and ensuring seamless coordination of care. My responsibilities include assessing and formulating management plans for acutely unwell patients, conducting ward round reviews, performing medical and minor surgical bedside procedures such as chest drains, lumbar punctures, and pleural aspirations, and assisting in departmental initiatives aimed at improving patient safety, clinical efficiency, and multidisciplinary collaboration.
In addition to patient-facing responsibilities, I have actively participated in quality improvement projects, audits, and educational activities, further enhancing my leadership and teaching skills. This experience has helped me refine my ability to prioritise tasks, make sound clinical decisions under pressure, and adapt to rapidly evolving situations. Through my role, I have gained confidence in managing complex cases independently while always being mindful of my limitations and escalating appropriately when necessary.
