Manage, coordinate and deliver care including the assessment, care planning, implementation and evaluation of the care for children with complex health needs and their families' referred to the WellChild service
Coordinate and where appropriate, expedite the discharge planning process for children and young people in hospital who have complex needs.
Provide intensive support for families' transition from hospital to home - Reduce readmission rates
