I am a committed and empathic mental health nurse with 23 years' experience working with adults/children/young people in both inpatient and community settings. I am knowledgeable and driven to promote the welfare and treatment of clients diagnosed with a variety of co occurring mental health/ eating disorders/ Neurodevelopmental conditions/SEN needs and trained in assessing and diagnosing ASC within N.I.C.E guidelines and Essence of Care Initiatives. I was previously the lead in safeguarding within a mental health community setting. I have worked with a cross section of ages, working with children/young people and adult services. I have established and run nurse led clinics and led in co occurring needs assessments, evaluating needs and charting individual treatment plans, ensuring good communication with clients to support understanding. This was to to ensure and promote holistic, individual care that met their needs, ensuring inclusion of equality and diversity within practice to reduce risk of social exclusion or marginalisation. I am excellent at working both independently and within a multi-disciplinary/ multi-agency team setting, maintaining strong links to children/young people/adults accessing the service, various external professionals and family members dependent on need. I strive in encouraging each service user to achieve the highest quality of life, appreciating the differences between people and treating peoples values, beliefs, cultures and lifestyles with respect. I have reflected and developed skills in approaches within inclusion and collaborative working throughout my nursing career.

Within my role as DCO i play a crucial role in supporting children and young people with special educational needs and disabilities (SEND). I have responsibility for coordinating services across the health and education sectors ensuring compliance with SEND legislation and advocating for the needs of children and young people. I effectively communicate and collaborate with various professionals including; SEN team, educational psychologists, therapists, social services, other health providers, education providers and parent carer forum. Within my role I focus upon working with partner organisations to consider the local profile of needs, quality of services and gaps in provision. I support a clear processes around mediation and tribunal procedures, where necessary. I work with the local commissioning groups to support their understanding of the needs of children in the local area and influence joint commissioning decisions. I attend variety of meetings within my role within the Quality Team these include (not restricted to) being the health professional on the multi agency EHC Assessment panel, I attend the DSD and Gateway Track meeting, Quality and Safeguarding meetings, I sit on the SEND Partnership Board and Annex A. Over the last few months of being in post i have been actively involved a series of projects including: lead for Partners In Neurodiversity In Schools (PINS), Out of Borough Quality Assurance for Education Placements, Preparing for Adulthood, review of Graduated Response for schools and assigned DCO lead for Neurodiversity Pathways, Mental Health and Eating Disorders. I have regular keeping in touch meetings with various professionals including (not limited to): Parent Carer Forum, Designated Social Care Officer, SEN case worker manager, Head of SEND and Associate Director, Children and Young People Services. My role includes education around SEND legislation and i have been involved with providing presentations for GPs, SENCOs and presenting for health care professionals.

Advanced Nurse -Autism and Learning Disabilities

As an Advanced Nurse for ASD/LD my role was care co ordinator for young people with mental health needs and additional NDP/LD/SEND needs. This included managing and providing clinical care in partnership with young people, their families and carers, education, social care, different physical health and mental health services and commissioners. The Advanced nurse role included assessment, care planning, multi agency working, treatment; including EMDR- therapy, ensuring paper work is up to date, risk assessments, safeguarding referrals and meetings, ensuring prioritising needs and risks identified throughout through effective time management. Within my role I actively engage in consultations within CYPMHS team and additionally for external mental health teams, i was involved in education for GPs and schools regarding ND/LD/SEND needs and reasonable adjustments. I additionally complete developmental/mental health assessments and reports to help with formulation of a young persons needs and advised if further assessment were supported and individual recommendations. Throughout the role i conducted ADOS assessments with the Neurodiversity Assessment Team. I attended schools and provide reports for EHCP plans for young people with SEN needs. I attended multi agency Transforming care/gateway meeting as the representative for CYPMHS and actively involved in discussions, presenting cases and updating the meetings. I attended SEND leadership self evaluation workshops and was actively involved in draft version of SEND INSPECTION SELF EVALUATION FRAMEWORK. I was part of the Autism Champions network. I provided supervision for colleagues within CYPMHS and student nurses.

Team Manager Neurodevelopmental Pathway Team

Within the role i was responsible for the pathway for the assessment of neurodevelopmental conditions with children and young people who require multi agency specialist consideration for their needs. Referrals include ADHD/ASD/GDD/SLT/Attachment. Young people referred may have additional co morbidities including mental health, learning disabilities, physical health and social needs. * Prepare and chair triage and outcome multi agency panel meetings. Meetings include speech and language therapists, community paediatricians, specialist paediatric nurses, specialist LAC teachers, educational psychologists, mental health practitioners and consultant psychiatrist. * Co ordinate pathway referrals/ assessments/ outcomes * Assess risks and prioritise/ refer to other agencies where appropriate * Work in partnership with health, social care and schools. Attend seminars and present to SENCOs, ADDvanced Solutions and GPs * Complete assessments for social communication difficulties: School observations, history taking- holistic needs and ADOS assessments * Support parents and young people on the pathway; advise and signposting to appropriate services for support. * Complete NDP panel outcome feedback's with parents/carers and complete outcome report. * Responsible for management and clinical supervision for NDP practitioner. * Attend management and clinical supervision. * Provide monthly statistics and report. * Attend operational meetings, steering group meetings, Quality and Performance meetings. * Complete and update SOPS, reviewing letters and communication. Children's Eating Disorders Service across Halton, Warrington, St Helens and Knowsley as a care coordinator. My responsibilities involved screening referrals and prioritising young person's needs within protocols and risk assessments. I assessed both urgent referrals and routine referrals. I was then actively involved within MDT meetings, feeding back assessments and discussing plans of care specific for client's needs, implementing and feeding back this information to the young person. I worked over 4 boroughs; Warrington, Halton, St Helens and Knowsley as an Autism Diagnostician with adults (16+) who are not diagnosed with a learning disability. My duties included triage, screening referrals, ASC initial appointments, ASC Assessments (ADI-R and ADOS-II), report writing and feedback sessions. Within the role i sign posted clients to appropriate agencies/treatments. I was actively involved in contributing towards the development of the ASC service protocols and procedures. I participated in audits and obtaining statistics for commissioners as required. I represented the service at ASDG meetings, within the meetings i was involved in reviewing and developing services available for clients with a diagnosis of ASC. I actively contributed and chaired ASC MDT meetings and forums. I supported new starters within the ASC service with regard to the utilisation of formal ASC assessment tools. I participated in selection of staff application forms and induction of new staff. I have delivered autism awareness training (LDTA) to health care and social care professionals and carers. Due to being diagnostic service only i compiled an autism awareness pack with reasonable adjustment information, which was sent out to professionals who are involved with clients diagnosed with ASC. I was in contact with various teams including mental health and IAPT teams to help them to formulate plans of care and therapy relevant for clients with a diagnosis of ASC. I was involved in formulating presentation for mental health teams. I had a case load which i managed and adapted to individuals needs and requirements. I was also the RIO lead within the team.
