This means I am able to assess all aspects of their health, mental health and current situations such as homelessness, addiction and poorly mainted healthcare needs and complex chronic illnesses. This creates a more positive approach to managing their needs as they are much more willing to liase with community services such as their GP, drug and alcohol rehabilitation programmes, homelessness charities and charities such as Project Nova.
