• Provide personal care and support to Clients with wide range of needs, illnesses and disabilities
• Know and understand care and support of client
• Undertake tasks detailed in client's care and support plan using person-centered approach and in least intrusive way
• Encourage independence and motivation of client and do not foster dependent behaviour
• Provide input into care and support plans of Clients by regularly feeding back to Management Team
• Assist Clients in getting up in morning and going to bed at night
• Assist Clients with toileting, continence management and personal hygiene
• Delivered high-quality care to clients with disabilities, achieving care plan objectives.

Key Responsibilities

• Ensure appropriateness and execution of business model and all operating processes
• Oversee caregiver in-house orientation and all training
• Liaise with outsourcing company on employment lifecycle of caregivers
• Schedule and conduct caregiver annual reviews and all supervision including regular performance conversations, accountabilities, customer satisfaction and problem resolution
• Motivate and inspire caregiver workforce
• Oversee caregiver schedules with emphasis on creating high-quality matches and extraordinary relationships
• Meet with prospective clients to discover their needs, sign them up for service and establish billing relationship
• Ensure satisfaction, safety and quality care through regular phone contact with clients and representatives as well as live visits in their homes
• Reflect values of PCA Home & Elder Care
• Be point of contact in clinic for clients and family members to facilitate problem resolution for client care issues
• Conduct client/caregiver introductions
• Keep sufficient documentation of all relevant phone and in-person contacts with clients, client representatives, caregivers and others
• Interacted with healthcare providers to manage patient treatments.

• Ensure clients receive their medication on time
• Support clients who can't move on their own
• Help clients dress, bathe, eat etc.
