- As a commissioned partner of Hertfordshire County Сouncil we launch the project which starts from support Ukrainian guests and extend to general refugee support
- I had a team of two link workers, and we became from beginning as service who work as a partner and should provide general support and information for guest
- But later after two month it becomes more wider support such as: mental health support, support with communication around GP, Citizen Advice Bureau, Housing, Befriending and community support, emergency accompanies, raised a safeguarding concern and also organized training around suicide prevention, understanding of safeguarding, recognizing common mental health disorders and self-awareness of burnt out and etc
- Key responsibilities: Provide robust support, advocacy, and facilitation for clients and their families within their local communities or chosen environments
- Conduct comprehensive assessments of client needs, meticulously preparing personalized reports with cost-effective recommendations
- Collaboratively establish goals with clients to advance their rehabilitation journey and life ambitions
- Efficiently organize and oversee care packages for optimal client support
- Research, coordinate, and monitor a spectrum of healthcare, rehabilitation, educational, and vocational services—both private and statutory—to ensure favorable client progress, outcomes, and cost efficiency
- Undertake thorough risk assessments and develop detailed risk management plans to safeguard client well-being
- As a project within our service, our main task was to build partnerships with other organizations and authorities, promote our service and managing the dashboard with 35 clients minimum per month
- Undoubly, we have achieved success in this
- Building trusting relationships with partners allows us to get referrals each day from such sources like HCC, CAB, HPFT, NHS, Home4Ukraine department, housing department and list of charities
- During 10 moths my team support 810 (including children 435) Ukrainian guests, 420 hours face to face support, dealing 80% mental health issues, two suicide prevention and 235 homelessness prevention and organized on weekly basis PTSD group talking therapy support in each community around Hertfordshire with the support of churches
- Even though the project was initially planned for 2-3 months of support, thanks to hard work we extended the project by almost a year.
