I worked within the therapy team for 10 months supporting the team with minor adaptations, equipment and identifying for major adaptations, and completed documentation overseen by the Occupational Therapy, Assistant Team Manager. I engage directly with the public, both over the phone and through home or care setting visits. My responsibilities are diverse and include assessing people's needs and conducting regular reviews, this could be through reviewing equipment or care plans. Often, I serve as the initial point of contact for the public with our service, collecting preliminary information and determining the need for further assessment, as well as directing people to additional services. In collaboration with individuals, their carers, professionals from other agencies, care providers, and Support Brokers, I assist vulnerable adults and their families in identifying and organising services that cater to their needs and enhance independence, choice, and control.
