I consider myself a hardworking, dedicated, and focused individual, deeply committed to my profession. As a medical professional, my top priority has always been providing excellent patient care. Since my graduation, wherever I have worked, I have consistently received commendations for my sincerity and dedication to my responsibilities. My colleagues and superior often praise my ability to create a comfortable environment for both patients and coworkers. Their encouragement has always motivated me to give my best. I find great fulfillment in my role as a medical professional and am eager to continue advancing in this field.
Currently, I am working as a junior doctor in East Lancashire Hospitals NHS Trust, which has two working sites (Royal Blackburn Teaching Hospital and Burnley General Teaching Hospital).

During this period, I've worked in the Neurosurgery department of United Hospital, Dhaka. This is a 500-bed multi-specialty hospital, occupied with the latest lab technologies, with premium healthcare facilities.
As a junior doctor in the ICU, here are my responsibilities:
I completed my internship at Mitford Hospital, which is a 1500-bed multidisciplinary hospital and also a leading teaching hospital in Bangladesh. It provides government-funded treatment to a population of around 4 million.
When I started my internship, it was peak time. COVID-19 had just begun to spread in our community. It was a very challenging environment to work in. But, I consider myself lucky enough to serve the people of my country during that time, one of the greatest pandemics.

#Description of Audit
An audit to ensure prophylactic antibiotic prior to flexible urethrocystoscopy in the outpatient department of Urology according to the
national guideline to prevent post procedure complication.

Aim & measure of the change
To prevent post procedure complications such as urinary tract infection in the patients undergoing flexible urethrocystoscopy by
developing a system to check the pre requisite before the procedure and educating the staffs regarding guideline.
Conclusions/Actions
Based on the result of first cycle of audit, the reason was found to be the lack of awareness of the staff regarding the guideline. After
implementing changes such as Cystoscopy checklist introduction and team meeting, a re-audit was done. It showed an improvement in
care from 75% to 94%.
#Description of QIP
Enhancing accuracy and efficiency in drain tube management; A quality improvement initiative for accurate labeling and output monitoring.

Aim & measure of the change
As none of the drains in post operative patients were labeled, these were creating confusion and affecting patient care. Also we collected data about wrong documentation of drains in the operation notes. The goal of the QIP was to make everybody aware about proper documentation on notes and labeling of the drains.
Conclusion
We have completed and presented the first cycle of this audit. We will be starting second cycle.

Pathogenesis of Tuberculosis (2018)
It was some of my fellow coursemates, found it difficult to understand how mycobacterium affects the lungs at the cellular level. So I made
some notes about the topic, took help from some microbiology books and also from the internet. I made a flowchart to make the topic
easier and delivered it to a group of 12 people. I received very good feedback about that session as it was very specific and well-
explained.
Examination of inguinal hernia (2020)
When I was doing my internship, I taught 2 final-year medical students how to examine the inguinal hernia, the types of inguinal hernia
and how to differentiate them. I was asked about management options for a hernia while teaching them. As I didn't have good knowledge
about that part, I acknowledged that and offered to discuss it in another session. Afterwards, I learned it and on another day I discussed
the management options with them.
Urinary catheterization (2023)
At my current workplace, I was requested by four nurses to teach them how to perform urinary catheterization. I demonstrated the
procedure and performed a catheterization in front of them in order to teach them. I received excellent feedback from the nurses and they
told me that they have not seen anyone perform urinary catheterization with such care and delicacy. After the demonstration, they were
also able to perform catheterization by following the proper steps.

To understand the pattern of most common surgical diseases occurring in adults and to assess the prevalence,associated co

The most common presentation was from patients with breast carcinoma (8.41%). Maximum morbidity among the respondents was
hypertensive (18.25%).

# Rota co-ordinator:
During my internship, it was my added responsibility to plan and manage the rota and maintain lias with my colleagues. They always
cooperated with me and were satisfied with my management.
# Class monitor:
I have been selected as a class monitor several times in my medical college, high school & primary school as well. I always tried to show
commitment to the responsibility vested upon me.
# Volunteering activity:
In 2015, I volunteered for a campaign in the election of North Dhaka City Corporation to help the voters find out the booths and cast their
votes.
# I worked in the DGHS (Directorate General of Health Services, a government health organisation) Corona Sample Collection Mobile
Team as a volunteer. The team collected samples from patients staying at home. I was a part of the organizer, who ensured to send lab
technicians reached the addresses, from where samples needed to be collected. Also, a part of my duty was to follow up with the
COVID-19-positive patients over the phone, who were staying at home.
# While working in the ICU department of United Hospital, I was a part of the resuscitation team. I took part in CPR & also perform a
significant role as a member of the team in managing critical patients.
# Internship was no exception. I sought help from my seniors whenever I faced any difficulty as it was when COVID 19 just broken out in
our country. Being a part of an efficient team, I managed the patients and provided health services to them as they required.
