As a locum GP I dealt with patients presenting with minor illnesses including viral infections, Allergies, musculoskeletal symptoms, and common infections of the respiratory system, ENT, and genitourinary system. I have been involved in the care of complex patients with cardiovascular conditions, endocrine and diabetes, ophthalmology, dermatology, and women's health.
At Special Allocation Services, I dealt with complex patients with behavioral challenges, Drug abuse, and complex mental health conditions. I performed home visits for patients at risk of self-harm and performed capacity assessments.
At Acute Respiratory Hub (Winter pressure Allocation), I reviewed respiratory patients with acute and chronic respiratory conditions including chronic cough, asthma, and COPD. I Managed these patients in collaboration with respiratory nurses in the community and worked for the intermediate unit for secondary care referrals for investigation and acute management where required.
Home visits of registered patients for acute health issues in elderly and palliative care.
At urgent care units and walk-in Centre, I provide unscheduled care to the patients who walk into the urgent treatment Centre and have appointments with out-of-hours GP service via 111
Face-to-face care for patients who access the service, when the UTC(Urgent Treatment Centre)is open Patients who present to the UTC will be walk-in patients or patients booked in by NHS 111.
Performed my duties as an autonomous clinician providing clinical assessment, diagnosis, and treatment (where appropriate), for patients that attend the UTC, either walk-in patients or those referred from other stakeholders such as the NHS 111.
Triaged & prioritized patients to elicit the history of an event/illness including past medical and drug history.
Evaluated clinical information from history and examination and initiated appropriate treatment, referral, or discharge.
Refer patients to appropriate care settings, where necessary including hospital specialties, hot clinics, DVT clinics, etc.
Discharged or treated, within the scope of practice, as appropriate.
Prescribed, and administered medication and treatments according to agreed protocols, within the scope of competence.
Maintained contemporaneous computerized clinical records of every contact, including history, examination, diagnosis, plan of care, advice given, treatment, and recommended follow-up. This included documenting any adverse event, safeguarding child or vulnerable adult issues or other notifiable matters.
Worked within the performance framework for the UTC service (key performance indicators set by the CCG for this service) Clinical Quality Commission and the National Quality Requirements for OOH.
Referred to other health and social care professionals and providers, as appropriate.
