Treating Tobacco Dependency Lead Advisor

To support implementation of the NHS Long Term Prevention Plan for Treating Tobacco Dependence within Secondary Care, working alongside AWP Lead Practice Development Nurse for Physical Health and Smoke-Free. I have been instrumental in supporting AWP Smoke-Free policy and Treating Tobacco Dependency implementation as per the national directives, for mental health services.

As the Tobacco Dependence Lead Advisor I have been responsible for the development and implementation of the new Treating Tobacco Dependence Service, initially for inpatient services.

Working Trust wide, initially covering the inpatient services within AWP, but with scope to extend services to those based under the care of the Community Mental health Teams in the future.

I have also been responsible for delivering and teaching evidenced based practice and develop maintain networks to share good practice and learning from each other across the trust and wider systems in BANES, SWINDON & WILTSHIRE.

Clinical Responsibilities

  • Provide a high quality tobacco dependence treatment service that is sensitive to individual's needs and in line with evidence based guidance
  • Act as a professional role model promoting evidenced based high quality patient care
  • Provide support for AWP staff in relation to their work with smokers
  • Provide opportunistic brief advice to smokers within clinical environments (patients, carers, staff, visitors)
  • Work autonomously in line with the AWP's Smoke Free Policy, NHS Long Term Plan, NICE guidance, and NCSCT best practice guidance
  • Deliver a confidential one to one, drop in and group based treatment for patients and advice for AWP staff who smoke
  • Complete comprehensive assessments of smokers needs, including the assessment and recommendations for the use of stop smoking pharmacotherapies
  • Facilitate access to pharmacotherapy in line with Trust protocols
  • Provide intensive behavioural support for smokers to support cut down or quit plans
  • Provide assertive in-reach to services to actively engage smokers in conversations about the benefits of stopping smoking
  • Formulate, document and evaluate personal stop smoking plans, in collaboration with smokers and their care team, and ensure continued support on discharge
  • Liaise with clinical teams regarding the safe and effective care for the management of prescribed medication(s) associated with stopping and restarting smoking
  • Act as a specialist resource for staff in relation to stop smoking issues
  • Develop, review and deliver learning to the highest standard and quality through different platforms and environments, including clinical practice-based work

· Build and maintain good working relationships with a broad range of internal and external stakeholders on issues relating to Smoke-Free and treating tobacco dependence

  • Plan, organise and prioritise own patient caseload and workload to ensure the demands of the service are met
  • Represent the trust as the smoke free expert at internal and external settings

· Maintain clinical professional registration (if post holder already a registrant) and specialist knowledge that is required to remain effective within the post.

Service evaluation

· Be instrumental in the identification, development and implementation of policy, procedure, guidelines and clinical audit in relation to local, national and professional agendas and monitor adherence taking appropriate action in line with Trust Governance structures

· Maintain high standard of record keeping of activity for monitoring purposes, in accordance with best practice guidance, trust policy and the requirements set out by NHS and the ICS best practice guidance

· Assist in data collection for service evaluation

Leadership

· Act as a role model and ambassador for the Tobacco Dependence Treatment Service

· Create and develop strong engagement and clinical expertise with staff and patients, to help grow the understanding of treating tobacco dependence in order to inform and measure future improvements across the Trust

Community Substance Misuse Worker

My role as a community substance misuse worker was to facilitate the recovery of substance misuse clients including alcohol, complex cases including mental health and poly drug use clients in Ceredigion. As part of that role I provided a triage of assessments including, specialist drug assessments, specialist alcohol assessment, risk assessments including risk assessment management plans & WARRN (Wales Applied Risk Research Network: clinical risk assessment and risk management training and consultancy).

Upon completing assessment I would discuss with the team the best course of treatment for the client, i.e. substitute opiate prescribing, alcohol prescribing and or continue offering 1-1 work around relapse prevention, while working within the NICE guidelines.

As part of this caseload, I worked alongside clients to draw up their care plans and empower clients on how best to move forward with their treatment goals, these care plans form a contract and can be extremely useful when used and reviewed every 3 months, along with the risk assessments being updated in line with service provision. The core part of my work is to provide harm reduction advice to clients and families and to minimise the impact this has on them and the wider public. This work can be in the form of safer injecting advice, reduction in prescribed medication advice, advice and support around reducing alcohol use, all of this work will be in coordination with the GP or specialist.

I have also assisted and led groups with clients & colleagues on psycho social interventions.

Part of that role was to understand and use the Trans-theoretical model of change to help support clients move forward through treatment. Through working with clients I explain the trans theoretical model of change and how it relates to them and their current circumstances or situation, the model includes Precontemplation (Not Ready), Contemplation (Getting Ready), Preparation (Ready), Action ( active modification of behaviour) & Maintenance(sustained change, new behaviours replace old), working through this can help conceptualize the process of intentional behaviour change.
