  • My duties and responsibilities were to reable service users after discharge from Hospital. Supporting with their daily tasks until they were able to manage independently or require ongoing care.
    My duties included:
    - Personal Care
    - Stoma and catheter care
    - Giving/prompting users medications
    - meal preparation
    - liaising with the patient to ensure a person centred approach
    - liaising with family members
    - completing daily care plans
    - assessing service users needs e.g. pressure sores, mobility, equipment
    - liaising with other health professionals
    - Keeping up to date with my mandatory training e.g. manual handling
    - Ensure wearing correct PPE following policies and procedures
    - Communicating with fellow colleagues via teams or when working as a double up team
