I have been an Outreach Worker for 5 and a half years, I love making a difference to families lives and ensuring they have the best chance in life.
In my role we receive referrals from other professionals such as health visitors, early help practitioners, midwives, mental health workers, G.P's, Schools, homeless families health visiting team, floating support, women's aid and many more.
The referrals will come through the North Early help allocations meeting where a number of professionals will meet up weekly and discuss each referral and decide which services the family will benefit from.
We also receive self referrals where the parent can come into the children's centre and ask for support face to face or they can do this over the phone.
Once I have been allocated a referral I will contact the referrer and ask if there are any risks with the family I should know about.
I then contact the family and arrange a home visit the first home visit is usually with another member of my time however the majority of home visits are lone visits.
I always ensure that when I arrive at the home I look for any risks and that I have a clear knowledge of the exits within the home and always have my mobile phone.
I will carry out a strength based conversation following the Early help Assessment form asking personal questions about the home life, social life, work/school/college life and health and wellbeing.
During the strength based conversation I intend to get a lot more knowledge of the family and the support they need.
As the referrer may have come in because the family feel isolated however when you get chatting to the parent it is identified that the family are new to the area due to domestic violence in there previous home and they have moved out the area.
Within my role I have an excellent understanding of safeguarding.
I am able to make decisions regarding safeguarding issues, I can assess each individual incident on a case by case basis, and work within the relevant policies and procedures.
I have previously raised safeguarding issues, as well as implementing new ways of working following procedural updates etc.
