Duties included:

• Carrying out patient observations according to hospital policy to keep patients safe from harm.
• Supporting patients with day-to-day tasks
• Assisting with personal care as necessary and promoting independency
• Being an assigned key worker to patients and encouraging them to be part of their care plan design
• Liaising with patients families or guardians to deliver a person-centred care
• Delivering a holistic approach of care to service users
• Working in collaboration with colleagues, the Multi-Disciplinary Team and external health care professionals

Roles and responsibilities included:

• Supporting individuals with tenancy sustainment issues
• Dealing with issues involving housing, benefits, welfare rights, personal finance, life skills and promotion of personal independence
• Handling matters relating to the tenancy such as Housing Benefit advice, antisocial behaviour and arears advice
• Dealing with issues that breach the tenancy agreement including harassment, nuisance and other disputes
