My main role with the district nursing team is to provide person centred care led tasks, these can tasks include:-


  • Medication management.
  • Ensuring at all times care plans are up to date and suitable for ever-changing circumstances. This also means ensuring each individuals care-plans are up to date and needs are being met within a timely manner.
  • Wound care, this can be a varied as monitoring pressure areas, skin tears, compression dressings, whist being able to identify the change in patients care or needs and adapt plans to suit this.
  • Risk management, I both understand and have the ability to identify potential risks, and act upon strategies quickly to prevent them from escalating.


This role has enabled me to pick up new skills daily, which allows me to to apply these skills quickly. Whist implementing them to my role to improve both productivity and efficiency. I carry out clinical and social care tasks often at difficult times in a home environment. I also keep up to date client documentation records, whilst complying with strict data protection guidelines at all times.

I joined the NHS in 2012 to work for the Supported Hospital Discharge Service. Which later merged in 2016 to become the Hospital Assessment Reablement team within my role as a reablement support worker was to provide clients with both long and and short term health issues with support. This includes such tasks as:-

  • Providing implemented client-centred reablement care plans with the goal of being able to maximize their independence.
  • Conducted regular home assessments to identify potential risks and hazards, and adapting individual care plans to suit there needs.
  • Regularly liaised with multidisciplinary teams politely and efficiently.
  • Assisted with clients to achieve person centred goals, whilst setting an achievable time frame.
