  • Keep detailed records of patient progress, assist medical staff in treatment planning. Daily use of Microsoft Office, Rio, Teams, Zoom, emails, digital diary, telephone call appointments.
  • Handle challenging behaviour from dementia-affected individuals effectively.
  • Sign Post patients or their families to support that they need. Escalate when needed to various teams, also raise Safe Guarding where necessary.
  • In the community, I am a lone worker and carry out Observations, such as Oxygen Sats, Blood Pressure, Weight checks, Temperature checks and safe home checks.
