  • As the Lead Practice Nurse working in large and busy General practice it has been essential for to be time critical, organised and flexible in my approach to the working day. I have a wide range of responsibilities which include the organisation and running of a variety of clinics within the practice for the surgery's practice nurses and healthcare support worker. I work within a multi-disciplinary team and attend weekly meetings to review patients, procedures, and policies so that the most appropriate services can be implemented. Clinics undertaken include the following:-
  • Immunisations - arrange, perform and recall patients to immunisation clinics for a variety of immunisations such as childhood immunisations, Flu, Pneumococcal and Shingles according to the UK immunisation schedule. These immunisations can be both age and time critical so organisational skills have to be impeccable and accurate at all times. It is also my responsibility to ensure that every immunisation has an up to date Patient Group Directive so that each immunisation can be given in a safe and legal manner. I currently work in a very culturally diverse area where patients have come from all over the world. It is important that I ensure that each patient is fully vaccinated according to the UK immunisation schedule. At times this can be very complex so it is vital my research and knowledge is extremely thorough.
  • Wound care clinics - I work with patients with wounds ranging from superficial cuts to complex wound management such as leg ulcers and skin grafts. Whilst always maintaining an aseptic non touch technique to prevent complications such as infection.
  • Chronic long term health conditions - I run clinics to review patients who suffer from long term chronic health conditions such as Dementia, Asthma, COPD, Diabetes and Hypertension. I review each patient comprehensively looking at their lifestyle, activities of daily living, physical observations, Venepuncture and medications. I will refer patients to appropriate services such as Occupational therapy, dieticians, podiatry, pulmonary rehab, secondary care and GP's if their medications are no longer managing their conditions effectively. This is to prevent the patients from needing hospital admissions.
  • Cytology - I perform primary HPV and cervical screening for all age appropriate patients who have been invited by Cervical Screening Wales.
  • Contraceptive clinics - I advise and review patients on a variety of contraceptive methods, Including the contraceptive pill, contraceptive injections and long acting contraceptives. It is imperative that I take a comprehensive medical history so that the patient can make an informed choice on what is the safest form of contraception for them according to the UKMEC.
  • Venepuncture.
  • HRT reviews.
  • Warfarin monitoring.
  • B12, Prostap and Nebido injections.
  • Point of care testing.
  • I complete accurate documentation for each patient per appointment. I always communicate clearly, honestly and empathetically with patients, relatives and colleagues with accurate evidence based information. Both verbally and through written documentation on a daily basis. It is also my responsibility to complete regular stock checks and ordering for immunisations and equipment and ensure that stock is rotated and in date.

  • As a Dementia care advisor for the Young Onset Dementia Team I worked both in a community setting and closely with the Young Onset Dementia ward. I co-ordinated the care for a case load of 50 patients and families, while maintaining high standard of professional care in a community and secondary care setting to ensure patients and their families could live and manage their health needs in a community setting whilst safe to do so.
  • I encouraged patients to keep feelings of independence and self-esteem by doing things for themselves and would implement resources such as care in the community, respite, activities, regular reviews from a multi disciplinary team including speech and language, occupational therapy, physiotherapy, social services and psychological support to ensure patient and families well being and safety .
  • I monitored patient behaviour and responses to treatments and reported concerns to senior staff including the team leader and Consultant Psychiatrist.
  • I recognised patient condition changes requiring intervention from doctors and other healthcare professionals to provide holistic care. I dealt with both medical and mental health emergencies with calm and composure. Putting care plans in place to provide support in the community and also admitting patients to a secondary care setting when required.

  • As an RGN working on a mental health ward specialising in young onset dementia my duties and responsibilities were varied on a day to day basis. At all times a flexible approach was essential to meet the unpredictable needs of the ward. I was employed on this ward to be able to utilise my medical based training to assist my RMN colleagues to be able to provide a more holistic based approach to nursing so that there would be less of a need to transfer patients off of the ward for medical interventions. My duties included: -
  • Pre-empting and assisting all patients with their activities of daily living whilst trying to encourage patients to be as independent as possible within all areas of this so that they could maintain these skills for as long as possible.
  • Complete accurate documentation for each patient per shift to record dietary/fluid intake, skin integratory and bowel and fluid output.
  • Carrying out appropriate and timely physical and mental observations and monitoring using my clinical judgement. Act upon these observation and the patients mental presentation to commence appropriate action such as checking blood sugars, performing ECG's, escalating concerns to senior nurses, GP's and Consultants, providing reassurance, de-escalation and distraction techniques to prevent agitation escalating, and administering PRN medications if required.
  • To recognise the deteriorating patient and providing immediate life support when required. Attend weekly ward rounds with the senior consultant and report any improvement/deterioration in patients presentations. Liaise with a multi-disciplinary team and refer patients to appropriate services such as dietetics, speech and language, physiotherapy and dental when required.
  • Administer prescribed medications in a timely manner, including tablets, liquids, injections and suppositories.
  • Communicate clearly, honestly and empathetically with patients, relatives and colleagues with accurate information. Both verbally and through written documentation on a daily basis. Be able to adapt my communication skills to patients needs through expression, tone of voice, eye contact and physical demonstration.
  • Complete extensive care plans and risk assessments for my primary patients and update these accordingly when required. Writing detailed reports on a primary patient to present to managers hearings and tribunals to determine if a patient should still be detained under a Section 3 of the Mental health Act 1983 or to establish is if it was more appropriate for a DOLs to be in place.
  • Provide palliative care to patients at end of life so that a patient had a dignified and comfortable passing, whilst also providing their families with emotional support and reassurance at such a distressing time.
  • Give a timely and accurate patient handover to colleagues at the end/beginning of each shift.
  • Take responsibility and ownership of issues that may arise relating to the patients care during their time being cared for on the ward and to resolve any issues in an appropriate manner.
