• Lead East and West Speech and Language Therapy Teams in the Adult Learning Disability Service
• Provide communication and dysphagia assessment and intervention at specialist level for adults with Learning Disability who may also have autism, dementia and organic mental health issues and meet criteria for the team.
• Manage a caseload independently underpinned by evidence based practice that abide with local and national policies and protocols including risk assessment and care plans
• Lead and engage the team in focus group tasks regarding service development
• Deliver individualised training to relevant stakeholders
• Ensure triage is completed by the team, referrals are allocated reflecting capacity and competency of staff and that assessment and interventions are completed in a timely manner
• Attend and contribute to professional clinical meetings; case conferences, CPA, safeguarding investigations
• Attend leadership meetings regarding information governance and policy
• Complete and support others to complete capacity assessments
• Liaise with external agencies e.g. social work, hospitals, education, interpreters
• Work with a diverse population in terms of intellectual ability, linguistic, cultural and racial diversity and understand the barriers to accessing health services
• Promote a Total Communication approach
• provide clinical supervision for junior colleagues
• Support colleagues with meeting team goals
• Design and evaluate audit data, ensuring this is clearly communicated to colleagues

• Providing support to adults with physical and learning disabilities at a day centre.
  Duties included:
  Key worker to a group of people, arranging reviews, writing care plans and risk assessments
  Delivering sessions to a group of up to 15 people of mixed ability e.g. drama, cooking and art
  Supporting people with personal care

Providing practical and emotional support to adults with learning disabilities living in housing association properties
Support people to manage tenancies
Support people to access their local communities
Support people to engage in preferred activities

out of long term institutional care into their own homes
