My role focuses on providing intensive support to individuals experiencing acute mental health crises, aiming to stabilise and reduce hospital admissions.
Crisis Resolution Home Treatment
Assessment and Intervention: Completing comprehensive mental health assessments, evaluating risk factors, mental state, and immediate needs.
Crisis Management: Delivering short term, intensive interventions to stabilise mental health crises, working closely with patients, families and other agencies.
Care Coordination - Developing, implementing, and reviewing person centred care plans, coordinating with multidisciplinary teams to support recovery.
Outreach & Support - offering a high level of support in the community, minimising the need for hospital admission whilst facilitating quick access to appropriate services when needed.
Psychiatric Liaison Team
Accident & Emergency support - Providing rapid mental health assessments and interventions with A&E for individuals presenting with acute mental health issues, including suicidal ideation and self harm.
Collaborative Care - Work with A&E and general hospital staff to manage patients psychiatric and physical health needs, ensuring holistic care and seamless transitions to appropriate community services.
Crisis De-escalation - Utilising crisis de-escalation techniques to manage acute symptoms, offering therapeutic interventions and planning onward care/ support.

Throughout this time I was based on a challenging behaviour unit where I am responsible for taking charge of a shift which has enabled me to develop my leadership skills. I have developed a wider understanding of the mental health act from working with patients who are detained under different sections. I have taken role of ‘named nurse' for patients who are admitted to the unit and have implemented care plans, assessed risks and carried out core assessments which assess a persons mental health/ behaviours over a period of time. I am required to chair regular MDT meetings for patients in my care and liaise with outside agencies in finding new accommodation for patients whose needs have changed and require a new care package. I carry out tasks with regards to the day to day running of the unit and keep records and documents accurate and up-to-date. I have been actively involved in introducing and driving service improvements in line with CQC requirements.
Working in a newly commissioned service offering a high standard of nursing care and support for people with learning disabilities/mental illness, challenging behaviours and other complex needs such as a sensory impairment or epilepsy. I was involved in marketing, clinical lead to staff nurse and support workers, managed duty sheets and supervisions. Carried out pre admission assessment; from these produced holistic care/ support plans to support funding applications.
Working on an assessment unit in Carlisle for patients with an organic illness, such as Dementia, providing a comprehensive and holistic assessment of the older person's physical, psychological, behavioural, social and spiritual needs through validated assessment tools and multidisciplinary working.
I was employed as a staff nurse on a locked rehabilitation unit for males who are detained (MHA). This is a specialist autism hospital providing a multidisciplinary approach to patients care. The hospital have adapted the ‘my shared pathway' approach where patients are involved in all aspects of their stay and discharge planning. I have been appointed named nurse role for two patients on the ward and I am responsible for keeping all care plans, risk assessments, positive behaviour plans, health action plans up date. I attend regular MDT meetings to discuss the care and progress of the patients and also produce reports for Mental Health Tribunals and Care Approach Meetings.

Since June 2013 I was employed as Acting Deputy Manager of the ward where I continued to carry out the above, working two shifts per week on the ward however also have additional responsibilities.
During this period I was seconded to a different service (Syra House) for patients with enduring mental health needs. This is a mixed sex unit for patients aged 18 years and over and offers a full range of investigative and therapeutic interventions, which were tailored to meet the needs of the patient. I was named nurse for one patient which required me to meet with the patient at least three times a week this was to discuss care planning, set goals and wishes for the future. This ward has been a long stay ward and at present is currently aiming to resettle all the patients into the community with adequate support, therefore a large amount of my time was spent working with individual patients assessing skills and working towards achieving skills.
