Graciela Louise Lane Yana

I am a dedicated healthcare professional with 12 years' experience in patient care and home healthcare, specialising in elderly care and complex needs including dementia, diabetes, and vascular conditions. I currently work on a busy surgical admissions ward and support the Same Day Emergency Care (SDEC) clinic, where I’ve developed the agility and resilience required for fast-paced clinical environments.

I bring extensive skills in infection control, catheter care, aseptic techniques, and diabetic monitoring, with a consistent track record of ensuring patient safety and comfort. I also support patients during diagnostic procedures, providing reassurance and assistance tailored to their individual needs.

I communicate compassionately and effectively with doctors and multidisciplinary teams, and my leadership qualities have helped boost team morale during challenging periods. For example, I initiated a peer support rota to improve shift handovers and reduce stress among junior staff.
