This position was an opportunity for secondment from the HCSW role I was in to support the District Nursing Team as ICT Coordinator. This time then allowed me to attend Management Program in my Trust and the Appraisal Training.

• I currently work as a Clinical Health Care support worker for Southern health
• I work 3 days a week
• My role includes advanced wound care, catheter care, bowel care
• Monitoring of wounds and pressure areas and sores and treating them as according to their individual care plan
• I write care plans, Do holistic plans, Braden and nutrition assessments
• I am up to date on all my mandatory training and life support training
• My role is demanding and at times can feel very pressured
• However, I manage my time well and have a system that work for me where as I know at the end of the working day I have completed each task given for each patient and if needed have followed up with referrals to GPs, Podiatry, Tissue viability team, and if needed request Multi-Disciplinary meetings for specific people who I feel need more input and care
• I Take my role very seriously
• In saying that I am also very light-hearted, with a happy demeanour and enjoy making people smile and feel uplifted
• There is nothing better than attending an elderly person who is in pain and feeling lonely and leaving them laughing and feeling like someone has cared and given them some joy and kindness for the time you were with them

• I am was running 3 clinics per week providing NHS health checks to the public
• This is a nationwide project assessing the risk of developing Cardiovascular disease over a 10 year period
• I provide the clients Q-risk score by taking height weight measurements, Blood Pressure, health, lifestyle and family history information and point of care testing for Cholesterol and Diabetes, in the form of a blood sample
• I then discuss their Q-risk score and offer guidance and advice and possible referrals to help them to achieve a lower risk of developing Cardiovascular disease

• This can be a sensitive subject and I make sure I treat each client individually, understanding each person’s needs and expectations, ensuring they leave me feeling positive and empowered, regardless of the outcome of their risk
