

I am an enthusiastic, highly motivated individual, always keen to learn new skills and enhance my existing knowledge and skillset. I enjoy being challenged both mentally and physically, and often strive to push myself out of my comfort zone to ensure I reach my full potential. I am an independent person, capable of working effectively as a lone worker, however I also excel when working as part of a team due to my bubbly, caring and compassionate nature. I have excellent communication skills, which I have found to be an integral element as a healthcare professional.
As a community staff nurse a key role is to care for patients primarily in their own home, however community nurses also visit patients in care homes, and regularly run outpatient clinics. Community nurses regularly attend multidisciplinary team meetings to facilitate discharges from hospital and arrange the necessary care plans and equipment required for a successful transition from hospital to home. Community nurses provide holistic, high quality care working closely alongside patients and their families to provide a person centered approach ensuring patients have maximum input in the care they receive helping them feel fulfilled and valued. Promoting independence is a key element of care delivery, ensuring individuals reach their full potential especially in their own home in turn minimising risk of admission to hospital. Community nurses have an extensive skillset carrying out roles such as wound care, venepuncture, catheterisation, syringe drivers, PICC line care, chest auscultation aswell as managing patients with a range of long term conditions such as diabetes, high blood pressure, heart failure and respiratory conditions such as COPD.
