  • Holding a case load of families with vulnerable/complex needs and helping to support them with issues such as- mental health/emotional health, domestic abuse, drug and alcohol, parenting, housing, home conditions, SEN
  • Being the lead professional on the MAP assessment plan to form a plan of support and working with muti-agencies to support the families- schools, health, housing, drug and alcohol team, police and other specialist services- CAMHS.
  • Running and delivering specialist groups to parents- Nurture parenting programme, gateway (domestic abuse group for women) in a group and on a one to one.
  • Direct work with children and young people to gather views and feelings to contribute towards the plan and support for the family. Work completed around anxiety, emotional resilience, mental health, support around education and school, family breakdown/conflict.
  • Supporting families with children with specialist educational needs (SEN). Liaising with Woodview and other health and specialist services- Addvanced solutions.
  • Contributing towards children's educational health care plan.
  • Safeguarding- following Halton Safeguarding model and levels of need framework- step up process and making referral to children's social care through the Icart front door referral process.
