• Support children, young people, and families with complex needs and challenging circumstances.
• Conduct assessments to identify family needs, strengths, and areas requiring intervention.
• Develop and implement tailored support plans to improve family functioning and child outcomes.
• Carry out home visits and provide practical and emotional support to families.
• Work collaboratively with schools, social workers, healthcare professionals, and external agencies.
• Support parents to develop positive parenting skills, routines, and effective boundaries.
• Monitor family progress and review interventions to ensure positive outcomes are achieved.
• Maintain accurate records and prepare reports for reviews, meetings, and safeguarding processes.
• Ensure safeguarding and child welfare remain central to all aspects of practice.

• Holding a case load of families with vulnerable/complex needs and helping to support them with issues such as mental and emotional health, domestic abuse, drug and alcohol, parenting, housing, home conditions, SEN needs.
• Being the lead professional on MAP (multi agency plan) assessments to form a plan of support to help the families.
• Multi agency working with health, police, schools, social workers, housing, drug and alcohol teams, CAMHS.
• Running and delivering specialist groups to parents- Nurture parenting programme, Gateway (domestic abuse group for women).
• Direct work with children and young people to gather wishes, views and feelings to contribute towards the plan and form the support for the whole family. Work completed around anxiety, self esteem, emotional resilience, education and school attendance support, family breakdown, parenting.
• Supporting families with children with specialist education needs (SEN). Liaising with specialist health services and specialist charities and other providers.
• Contributing towards children Educational Health Care Plans.
• Knowledge of safeguarding and the process to support children within the organisational framework- step up process and completing referrals through the front door referral process.
• Supporting young people and families post 16 to support further education and training.

• To support vulnerable and disadvantaged young people who have complex needs or multiple problems and experience barriers to progression.
• To act as a lead worker and be responsible for a defined caseload.
• To provide individualised support to young people which are appropriate to their individual needs and level of maturity.
• Working within a multidisciplinary team to help reduce the negative impacts of substance, reducing the rate of teenage pregnancy, improving the sexual health awareness of young people, preventing first time offending and re offering, increasing the number of people in education or learning post 16.

Within my role I supported clients face to face, over the phone and by email dealing with a number of enquires from general job searches, supporting with CV writing, enrolling on to further education and learning courses and more in depth work with clients supporting them in group workshops and getting them work ready.
