Summary

8 years of Social Work experience and working as a social worker with the London Borough of Harrow in the Children and Young Adults with Disabilities Services 0-25 and have worked on Transition children to adult pathways within the service. I have a comprehensive knowledge of social justice issues and systems affecting families and children with special needs. Coming from a diverse country and culture I have been able to assist and have an overarching niche in the community of Harrow. I have had the experience of working with complex children and families who fall under the remits of CIN, CP, CLA, COPDOL and SEN Residential.

  • Working with children and young adults with disabilities and their families – severe physical and learning disability, sensory impairment, profound multiple disabilities, complex and severe health needs.
  • Preparation of Transition pathways and plans for children turning 18 within the service.
  • Children attending special Schools with an Education, Health, and Care Plan.
  • Undertaken Children and Families Assessments (Social Work Assessment), Parent Carers Assessments, Young Carers Assessments, Initial Viability Assessments.
  • Child centered and strength-based work engaging with Children Looked After (in foster placement, residential placement), Process of Full Care Order, Child Protection Plan, Child in Need Plan, Service Delivery Plan.

  • Sign positing children and families to appropriate services in Harrow as per the needs assessment.
  • Evidence based Court paperwork as appropriate.
  • Attended in house as well as external training programs and workshops on topics like Signs of Safety, young carers assessment, Adverse Childhood experiences (ACEs) – Preventing early trauma to improve adult health, etc.

  • Child Rights & Education Based at the Head Office of the Organization, I was responsible for the applicability of the programs mentioned below and ensuring its effectiveness at the grass root level by being supportive to the project teams and maintain centrally the records of all programs happening in all the projects and serve as a resource for the project staff.
