  • GENERAL MEDICINE

In my General Medicine rotation, I collaborated with colleagues to manage diverse cases within the department. Each day began with a morning review, where new admissions and management plans were discussed with senior doctors. I was then assigned to either ward rounds (male/female medical wards) or outpatient care, typically working alongside at least two other doctors, coordinated by the most senior doctor. On Mondays and Thursdays, we conducted grand ward rounds with consultants, often lasting 4-5 hours depending on patient volume. Outpatients varied by day, covering specialties like cardiology, neurology, and gastroenterology. As the first on-call, I managed emergencies, including diabetic crises, myocardial infarctions, and heart failure, creating immediate care plans. I independently performed procedures such as paracentesis and femoral cannulation and assisted with liver biopsies.

  • PAEDIATRICS

During my three-month rotation in Paediatrics, I worked closely with colleagues and consultants on a range of pediatric cases, from malaria to febrile convulsions and severe dehydration. I developed management plans, provided daily updates to consultants, and referred patients to tertiary facilities when necessary. I counseled parents on their child’s progress, addressing concerns, complications, and follow-up care. I managed medical inpatients and outpatients, performing procedures such as IV cannulation, lumbar puncture, and neonatal resuscitation, and independently referred complex cases.

  • GENERAL SURGERY

In Surgery, I communicated daily with consultants about post-op and new cases, monitored post-op patients, and assisted in various surgeries, including laparoscopic and emergency appendectomies. A typical day involved ward rounds with consultants or colleagues, assessing patient progress, and handling post-round tasks. I managed patients with conditions such as intestinal obstructions and burns. On night shifts, I attended to emergencies in both the ward and A&E and was involved in clinical audits and monthly presentations.

  • OBSTETRICS AND GYNAECOLOGY

In my Obstetrics & Gynaecology rotation, I managed diverse cases with colleagues, covering pre-conception, antenatal, postnatal, and gynecological care. I reviewed new admissions in morning handovers and handled ward rounds, discussing unstable patients with consultants and managing outpatient care. I provided patient counseling on fetal health, managed cases such as ovarian cancer, miscarriages, and labor complications, and assisted in surgeries like hysterectomies. As the on-duty medical officer for 3-4 night shifts weekly, I handled Obs & Gyn emergencies in the ward and A&E, conducted monthly presentations, and taught junior colleagues.

TRAUMA & ORTHOPAEDICS

In Trauma and Orthopaedics, I independently managed cases and worked with the team, serving as the first on-call for department reviews. I handled three weekly ward rounds, saw outpatients, and prepared patients for surgery, assisting in procedures such as fixation and amputations. I managed trauma patients in A&E, stabilized cases, and performed fracture management tasks like splinting and applying POP.

ACCIDENT AND EMERGENCY (A&E)

As the first on-call in A&E, I led a team of medical professionals to stabilize patients and coordinate emergency care. Each shift began with a team meeting before handling cases such as shock, cardiac arrest, trauma, and severe asthma. Working 24-hour shifts every three days, I led triage efforts and ensured all patients received timely care, referring those needing specialized treatment.

ANAESTHESIA

In Anaesthesia, I administered local anaesthesia, observed general and spinal techniques, and assisted with intubation and ventilation. My responsibilities included monitoring vital signs, managing IV fluids, and handling preoperative tasks such as urethral catheterization.

  • worked as a medical intern where I had 3 months rotations each in the departments of Medicine, Surgery, Pediatrics, Obstetrics, and Gynecology. During each rotation, I was the first on-call who attended to all patients who came into the various departments.

GENERAL SURGERY:

• My typical day during rotation in the general surgical department involved doing a morning review to acquire updates on new patients’ admissions, the current state of patients in the wards, to be aware of any unstable patients so that appropriate handover is done to doctors and other medical teams who would be taking over patients care and management from the night shift.

• Different routines are carried out within the week. For instance: Mondays are for grand ward rounds, Tuesdays are for Clinic, Wednesdays are for Major surgeries, Thursdays are for day-case surgeries. I Attend to patients that require emergency treatments and trauma cases within the A&E and the surgical wards.

• As the first point of contact for patients, I make initial diagnoses and management plans. I request follow-up and review investigations, then discuss the outcome with the registrar or consultants. I assist in the patient pre-op assessment, perform post-op instructions and monitor the post-operative care of patients. I perform blood transfusions for patients that require blood transfusion, manage blood transfusion reactions, and prepare operative lists and discharge summaries. I assist in surgical procedures like tumour excision, hernia repairs (15), exploratory laparotomies (10), laparoscopic surgeries (4), and Appendectomies (10).

GENERAL MEDICINE

• During my posting in General medicine, I worked with my colleagues to manage various cases within the department.

• A typical day involved a morning review where the consultants and junior doctors discussed cases from the previous day and night. Subsequently, a ward round where all inpatients are reviewed, adjustments are made where needed, available investigation results are reviewed, and new investigations are requested where required. After that, outpatients were seen in the clinic depending on the sub-speciality clinic for the day (either cardiology, neurology, gastroenterology, nephrology, or endocrinology).We were called to review patients in the emergency ward whenever needed.

• I managed life-threatening emergencies like acute exacerbation of peptic ulcer disease, acute exacerbation of asthma, myocardial infarction, heart failure, hypertensive urgencies, anaphylaxis, epileptic fits, and diabetic emergencies (DKA and HHS), hypoglycaemia and unconscious patients then make management plans for these patients. Continually, I manage the patients plan and discuss each patient with my registrars, senior registrars and consultants on duty where necessary.

• I performed some procedures like paracentesis, and femoral cannulation, and observed and assisted in liver biopsies. About 60% of my duties were carried out under supervision and 40% initially supervised and subsequently unsupervised. I participated in the monthly departmental meetings and the monthly presentations.

OBSTETRICS AND GYNAECOLGY

• During my rotation in Obs&Gynae, I worked alongside my colleagues to manage various cases within this department. My role involved attending to outpatients in the clinic, reviewing investigation results and advising accordingly, counselling patients on the condition of their baby inutero and anticipated complications where required and the management plan in place for such anticipated complications. Also, I worked with the midwife to monitor and support pregnant women in labour and managed complications, if any.

• A typical day involves taking over from the night shift and doing a ward round, discussing unstable patients with the consultant and carrying out discussed plan. I also support the outpatients’ clinics, this depends on the clinic day either Antenatal, postnatal or gynae. I review and interpret results then counsel the patients accordingly. Discharge patients fit for discharge with written discharge summary.

• I managed various cases such as pre-eclampsia, eclampsia, abruption placenta, threatened abortion, post-partum haemorrhage, obstructed labour, normal vagina delivery, caesarean section, UTI in pregnancy, leiomyoma, ovarian cancers and many more with other colleagues and gave updates to consultant in charge. I also carried out pre-op assessments and post-op plans for surgical patients.

• I assisted in many elective and emergency caesarean sections, hysterectomies, and myomectomies, carried out a series of evacuations of retained products of conception and managed labour complications. I worked three-night shifts a week as the medical officer on duty managing Obs & Gynaecology emergencies. I was supervised in this posting and carried out some procedures unsupervised

SURGERY (UROLOGY & PAEDIATRIC):

• I also rotated through urology and paediatric surgical unit where i worked closely with other medical house officers. I spent two weeks each in the two different sub-specialities.

• A typical day in the two different sub-specialities involves doing grand ward rounds, carrying out post ward round instructions, taking blood samples when necessary, attending to emergency cases (occasionally unsupervised).

• During my rotation in urology, i managed several cases of acute urinary retention(unsupervised), urethral catheterization(5) per day. Observed and assisted in prostatectomies, orchidectomy, prostate biopsies. During my two weeks rotation in paediatric surgery, i did pre-op and post op assessment, worked up paediatric patients for surgery, counselled patient's parents/family members about their concerns, conditions, complications, prevention and follow up plans before their discharge or once stable.

• My daily routine involved grand ward rounds, monitoring of post-op patients and managing post-op complications, attending to emergencies in the ward and in children emergency and reporting to my seniors as at when due, requesting, following up and reviewing investigation results, formulate appropriate plan and also review with my seniors.

TRAUMA & ORTHOPAEDICS:

• I was the medical house officer in the Trauma and Orthopaedic department for one month during my three months posting in the surgical department. I actively worked with the department's registrars, senior registrars and consultants to manage both inpatients and outpatients.

• I worked in the Accident/Emergency and Orthopaedic wards to attend to patients, request, follow-up and review results. I managed several cases within Accident and Emergency, such as closed and open fractures, hip dislocations, knee dislocations,pop application and ankle sprains.

• A typical day in the department involves doing ward rounds with my seniors’ colleagues, carrying out post-ward round instructions, attending to emergencies within the ward, taking blood samples when required, attending to emergencies in Accident and Emergency, and giving updates to my senior’s colleagues. I observed and assisted in the following surgeries: external and internal fixation (10), hip replacement (5), knee replacement(5), amongst many others.

ACCIDENT AND EMERGENCY

• I had a two (1) month rotation in the accident and emergency department where i managed various medical and surgical emergencies(such as road traffic accidents, lacerations, Diabetic emergencies, Hypertensive emergencies, comatose patients, head traumas, fractures, acute abdomen, cardiac arrest and many more).

• I was the casualty officer on duty who was usually the first doctor on call to attend to emergencies and stabilize them then invite necessary departments to come and continue management. I worked first under supervision and then unsupervised to manage and stabilize emergency patients both during normal working hours and during night shifts.

• A typical day in A/E involves attending to various emergencies and triaging where necessary, thereafter reviewing the patients with my seniors and inviting necessary team to take over management where necessary.

4. Teaching round- the managing consultants while on the ground ward rounds discussed with the junior doctors cases seen and their line of management.

5. As a medical house officer, I occasionally teach the medical students how to carry out some basic procedures like general and systemic examinations among other topics
