Leonard Cheshire support worker - providing personal care and support with everyday activities to children and families, older adults and service users with physical disabilities and learning disabilities.

Care UK community support worker - Assisting adults with eligible social care needs with daily living activities, including supporting with personal care, cooking and preparing meals.

Age UK - I supported vulnerable adults including older people and service users with physical and or learning disabilities and service users with mental health problems to live independently in their own homes. My duties included , ensuring that service users were able to access statutory and health/social care services, leisure and social activities. Making appointments and accompanying  service users to GP, dental and hospital appointments. I assisted with budgeting skills and debt management. I supported service users with making benefit claims including Attendance allowance and community care grants. I assisted service users to move into new accommodation.

CAB mental health student social worker - My main duties included debt management and advice, support with benefits, advocacy, housing issues, work related issues. I also supported service users at various meetings, including advocacy for voluntary patients with mental health problems and patients detained under the Mental health Act. I liaised with social workers in the mental health team, Sheffield homes, DWP, hospital ward staff including the hospital discharge team.

Derbyshire County Council Student social worker - I worked in a multi disciplinary team supporting vulnerable adults 18 years and over. I was responsible for the completion of assessments formulation of care packages and the monitoring and reviewing of care packages. I sign posted to other agencies when service users did not meet Derbyshire County Council's eligibility for services. I also completed assessments and reviews with service users in residential care homes. I worked with complex cases I carried out risk assessments and initiated safeguarding procedures when necessary. as a student social worker I participated in the staff duty rota twice a month. I attended multidisciplinary meetings including hospital discharge meetings. I organised and chaired review meetings with service users, their families and other agencies.

Nottingham County council Social worker - Agency work at Bassetlaw hospital - I was responsible for assessing patients' care needs upon hospital discharge. I worked in collaboration with patients, their families and other professionals  I attended weekly multi - disciplinary team ward meetings. I completed continuing health care checklists, attended SDT meetings and I completed mental capacity assessments.

Sheffield City Council - I complete Care Act assessments, support plans, section S42 enquiries, Mental capacity assessments, Chair best interest meetings, Complete checklists and attend DST meetings, complete court of protection work, including providing witness statements for court and attending court hearings. I support student social workers, social care practitioners and level one social workers.
