I have enhanced my clinical skills by working across a variety of Health Centres and GP practices, providing best care to patients respectfully, ethically and keeping the GMC standards up front. I provide comprehensive assessment, arrange appropriate investigations using resources properly, prescribe according to local and national guidelines and promote health. I understand the importance of good record keeping and communication when referring patients to colleagues and get involved in learning and teaching as much as possible.

worked in a busy urgent care centre as a Solo GP delivering effective medical care to a diverse population and making autonomous decisions confidently for both the walk in patients and DVT service. Triaging calls from 111, from district nurses, prison service and dealing with acute blood results.

Provided out-of-hours care to patients in Chorley and Leyland area.
Conducted telephone triage, face-to-face consultations, and home visits.
Supported district nurses, palliative care team, and ambulance services in delivering patient care.

Provided high quality care to patients of all ages and managed both acute and chronic conditions while promoting health. During this placement I was involved in enhanced service for elderly care and created care plans for all Nursing and residential patients in the list.

Maternity break following birth of my second child Feb 2011-Aug 2011

worked as speciality registrar in Primary care and provided consultations, home visits, baby clinic and attended out of hour session.

Worked in Paediatrics and Psychiatry Department. Focused learning on care of children including safe guarding and completed child health Promotion. Gained experience in assessment and management of patients with mental illness and suicide risk assessment.

Worked in Acute Medicine, Elderly care Medicine and Accident and Emergency. During this placement gained knowledge and developed skills to assess patients in acute illness, management of emergencies and needs of older population.

4 months placements in Gynaecology, Acute Medicine and General Practice. Developed skills in team working and coordinating care of patients in Primary and Secondary care.

4 months each placement in Breast Surgery, Urology and Endocrinology. I carried out day to day duties efficiently in the ward while learning eagerly and applying the skills. I attended the theatre sessions and out patient clinics along with ward work.

Observed Geriatric consultant in old age ward and learnt about clinical as well as social issues involved in care of elderly. I completed an audit on discharge delays of patients awaiting social services input. It sparked discussion at organisational level.

Attended out patient clinics and MDT meetings in Psychiatry Department gaining understanding of NHS and Mental Health Act.

I completed two months clinical attachment gaining experience of NHS. I attended ward rounds with the Gastroenterologist as part of the team and had opportunity to attend the endoscopy sessions.

Worked in Medical Ward clerking patients and attending ward rounds with day to day jobs.
