daily tasks to support with cleaning shopping and also managing with appointments and bills. giving emotion support following all risk and support plans i have cared and supported Individuals
of all different types of back grounds from Learning disability, mental health to dementia, working in Supported living and residential. While I am Still Currently employed at Knoll House, Edgeview Homes as a Support worker supporting people with Different types of needs from Mental health, Learning Disability and other health related needs. Supporting with Daily task and to Promote independence and Confidence to live independently in there own accommodation. Working in a Person centred way listening and understanding the needs of each individual and providing a high standards of care. Following support plans and Risk assessments, PBS plans giving accurate recording in there daily notes and supporting sitting in on handover's and also having meeting's with the people we support that's present with professionals following care legislation and company's policy and procedures and protocols.notifying management on any changes in support and risk plans. Supporting the people we support on health related appointments and offering alternative choice of activities that will benefit there well being, advising and encourage on exercise and prompting a healthy balanced diet and to make sure that they have hit there target on fluids to keep themselves hydrated. the reason to why i want to leave as this service is mental health and that I'm more of a fit supporting people with Learning disabilities and complex needs in what i enjoy doing..
Whilst I was employed with dimensions group I supported three young guys with down syndrome and autism. supporting them in a person centred way and in there best interest,shopping, medication.
and to make sure there medication as been ordered in a timely manor for the next months cycle. Supporting in meal Preparation
and encouraging a healthy balanced diet by giving alternative choices. Supporting the individuals with there finances choices of activities and appointments. liaising with management and professionals on changes. following Support and risk assessments
complying with Care legislation and company policy and procedures and making sure all there daily notes was accurate and up to date. the reason to why i left as i wanted to be close to home
and the structure of shift patterns changed. there was around an eight week wait for me to start my employment at Edgeview
as i was waiting for by references and DBS. in why there would be a gap in employment.
Whilst i worked at Highfield house Midway care group i supported
individuals with learning disabilities, Complex needs and working in a person centred way in there own best interest an providing a standards of duty of care. Supporting with personal care, GP/ Hospital appointments and other health Related appointments.
maintaining and implementing a healthy balanced diet of choice and encouraging on pushing fluids that they keep themselves hydrated. using picture books,basic Makaton and objects of reference of choice.following support and risk assessments

the reason to why i departed from the company as that i was only supposed to be back working in walsall temporarily as i was waiting for the company to open a new service close to my home
but due to health and safety CQC stated that the home was not
fit for purpose at that time as this was being pushed back got offered a position working with dimensions closer to where i was living.
While i was working and supporting Individuals at Priory Adult care supported living I supported the Individuals with learning disability and complex care needs that also had physical disabilities. Supporting with personal care, oral hygiene and providing a duty of care at all times supporting with medication and to make sure that this was ordered in a timely manor for the next months cycle. booking in and returning on unwanted medication also Supporting the Individuals on choice of meals and encouraging a healthy balanced diet and pushing fluids to keep themselves hydrated. following support and risk assessments sitting in on handovers and notifying management on any changes

the reason why left Priory Adult care was because i was offered a Position as a support worker with Midway care group and i knew how the company operated and knew me from past experience.

learning, and physical disabilities with complex needs, Supported using hoists where all my manual and theory training was all up to date. Supporting the individuals with personal care maintaining dignity and respect at all times. supporting with meals of choice

While working for midway care group at Highfield house this was my first time working and supporting the people that we support

While working for Steak rattle and roll as an assistant waiter/bartender meeting and greeting the customers making them feel welcome and introducing new offers and deals. the reason to why i left I got offered to work for Midway care group as i wanted to be support worker supporting people with learning disabilities and difficulties.
While working at Leonard Cheshire disability i was a care assistant Supporting the residents with physical and underlying health conditions and also end of life care caring in the last dying days. Making sure there Dignity is kept at all times interacting with them giving choices of meals and drinks following dietary requirements risk and care plans. providing a standards of duty of care and working in there best interest. caring in personal care, toileting, incontinence choices of activities on what they enjoy doing. making sure there daily notes are accurate and up to date

i was out of work for around eight weeks as waiting to start employment at Leonard Cheshire Disability on waiting for references and dbs.
While Working as an assistant hair stylist i was Studying my NVQ
level 2 in hair dressing where i had an apprenticeship learning cuts and colours and looking after the clients offering drinks,magazines and letting the clients know on latest offers and deals on Hair products. the Reason to why I left was i wanted to always work in health and social care but as i wasn't old enough at the time
and was offered a position at Falling's park lodge.
the skills and Knowledge that i have gain are my NVQ level 3 in health and social care understanding my knowledge of care legislation and supporting the people we Support in a person way
I have also Completed my Care certificate and all my training is all up to date
