Key Responsibility: Supporting care leavers transition from children to adult

• link in with Looked after children approaching adulthood from teams that support them(e.g. Asylum children seeking team, building your futures team) to build a relationship with them and prepare them for the transition to SYF team.
• Restoratively identify strengths and needs of care leavers and support them with available resources to address those needs which typically includes, housing, education, employment, medicals benefits and social relationships
• conducting risk assessments, wellbeing plan and developing pathway plans for care leavers.
• supporting care leavers through education and employment
• visiting care leavers periodically dependent on their age and assessing their development into transitioning to independent adults.
• Taking part in duty task which typically involves attending urgent situation such as supporting care leavers when they have to move accommodations, receiving duty calls and emails and addressing the issues raised which typically comes in a wide array.
• attending multi agency and inter professional meeting such as MARM
• Managing my caseloads effectively with the use of Microsoft Teams calendar
• Evidencing my work with the use of case note, care leaver contacts reports and other form of report writing.

Key responsibility: Supporting children and young adult with SEND to meet their needs in lie with their care plan and have a sense of belonging within community

• ·Assisted with personal care activities such as washing and dressing, consistently promoting positive health and hygiene.

Key responsibility: Support families restoratively with children under the age of 5 with a variety of needs;

• Supported children under the age of 5 to get ready for when they begin school vis a programme called Big Hope Big Future
• Coordinated Stay and Play group in families Hubs across the city for children under the age of 5 and their families.
• Supported families with filling and mailing DLA forms for their children with disabilities
• Provided emotional support for parents with children under the age of 5
• Managed my case loads and work with the use of Microsoft Team calendar
• Attended several meetings aimed at fostering development for children such as the Portsmouth parents board meeting.
• Evidence my work via case notes and reports on charity logs.
• Answered phone calls at the office and performed several other administrative task

Child Protection office (December, 2019- September 2023)

• Embarked on rescue mission on vulnerable children at risk of harm or experiencing harm
• Facilitated services rendered to children and other children matters in the Lagos State Family Court
• Carried out home assessment and social investigations on all allocated child protection cases
• ·Cleared and recommended children in care for adoption, fostering, guardianship or release to a fit relative all through the State Family Court
• Created awareness and sensitized the general public on child protection issues and the Lagos State Child’s Right law.
• Personally, handled the Lagos Sate Child Protection Unit Hotline for two years, thereby receiving calls from the public and attending to their complaint and information reported on child protection concerns.
• Collaborated with other stakeholders in the business of child protection in the State such as the Police, NGOs, Community leaders, Faith Based Organization and other Ministries Department and Agencies
• Compiled statistics of Child Protection Unit on a monthly, quarterly and annual basis.
• Took party in the quarterly supervision of over 70 Homes that sheltered children in the state with an audit of all the children's files
• Prepared speech, keynote address and presentations on child protection issues for permanent secretaries, commissioners and head of departments within the Ministry
• Attending to clients, individuals and organisation that walked into the child protection unit to enquire or report anything concerning child protection.

Human Integration Unit (May, 2018- December 2019)

• Embarked on after care and follow-up exercise through effective monitoring for adopted children by visiting children in their adoptive homes
• Carried out monitoring and assessment of children released from the State Correctional facilities.
• Monitored and Assessed persons released from the State Mental Rehabilitation centre in view of assessing their integration into the society
• Recommended released wards and rehabilitates in states for empowerment programs
• Prepared the Unit statistics on monthly, quarterly and yearly basis.
• Developed regular reports on the activities of the unit as required.
