
My role at the 700 Club as a Support Worker was to deliver housing-related support to clients aged 18 and over with hogh support needs. I empowered clients to develop skills and stability required to transition into more permanent accommodatipn and achieve greater independence. I key worked 8 clients and undertook essential administration tasks such as risk assessments and support plans. I supported clients with addressing hpusing, health, and wellbeing needs whilst promoting independencr and resiliance. I also covered shifts at 700 Clubs hostels and supported clients struggling with substance misuse promoting harm reduction stratergies and where appropriate facilitating access to spe ialist services.
My first role with Creative Support was as a Support Worker within a project supporting service users with complex needs and challenging behaviours. I moved to a second project which was a move on project where i supported clients with mental health, complex needs and learning difficulties. I provided person centered support to service users including short-term support to help them regain their independence and supported clients with complex needs who required intensive support to enjoy a good quality of life. I delivered individualised, person centred support ensuring service users led meaningful lives and helped them to reach their full potential asnindividuals.
