ST2 posting started with 6 months in both Dicconson Group Practice and paediatrics

• Worked with colleagues in paediatrics as first on-call clerking patients and instituting initial management plans.
• Managed patients at the GP surgery under supervision of the clinical supervisor- helping to understand and build on the use of available resources in care of patients.

The last 6 months had me doing rotations in psychiatry and Sivakumar and Gudes Surgery

• Continued to gain experience in managing patients and case loads/ tasks in primary care.
• Liasing with my colleagues in primary care for patient management- further understanding referral pathways in the community.

My first posting as a GP trainee was in orthopaedics where I spent 6 months understanding the workings of the NHS

• First on-call; clerking patients and taking referrals from colleagues in A&E and other specialties.
• Making preliminary diagnosis and requesting needed blood tests
• Stabilising patients and getting them ready for surgery- ensuring that they were optimised after reviewing their blood results

My next posting was in accidents and emergency where I also spent 6 months

• Managing emergencies in A&E
• Understanding referral pathways and NICE guidelines on management of emergency conditions that could present first in the GP surgeries

• Worked as a non-training clinical fellow providing care in the neonatal intensive care unit NICU to ill neonates.
• Attended deliveries and conducting neonatal resuscitation to neonates; also provided care for a wide variety of neonatal medical conditions. Completing the NIPE.
• Provided direct patient care in collaboration with physicians and nurses to develop an appropriate plan of treatment.
• Performed various procedures such as cranial ultrasound scans, ABG and IV cannulations, etc.
• Requested for relevant investigations such as blood test, x-rays and CT scans as well as prescribe medications within scope of practice according to accepted standards of care.
• Involved in non-clinical work such as audit, quality improvement and teaching of medical students, and other health care staff.

• Worked in the neonatal intensive care unit NICU caring for sick neonates. Attending deliveries and participating in neonatal resuscitation; IV cannulation, history taking and physical examination, NIPE etc.

• Assisted the consultant in performing surgeries like caesarean sections, myomectomy, exploratory laparotomy, herniorrhaphy, appendectomy etc.
• Ensuring that patients are well prepared before surgeries, including reviewing investigations done prior to surgeries, and communicating with consultants on patients’ preparedness.
• Management of common childhood illnesses like meningitis, malaria, upper respiratory tract infections, urinary tract infection, pneumonia, diarrheal diseases etc.
• Gave appropriate health education and advice to patients and their caregivers to enable smooth self-care upon discharge.
• Proficiently performs venepuncture, lumbar puncture, intravenous cannulation and urethral catheterizations.

• Ensured acute management of emergency medical, surgical and trauma cases; assessing patients' needs through history taking, examination, investigation and treatment with minimal or no support from seniors before inviting various specialties for a review. Cases seen include severe acute asthma, acute exacerbation of COPD, burns, shock, seizures, acute abdomen, and various degrees of trauma.
• Provided inpatient and outpatient medical care to patients according to hospital protocols and national standards.
• Implemented minor lifesaving medical and surgical procedures like primary survey on multiply injured patients, fracture stabilization and splinting/casting, wound suturing, intravenous cannulation, venepuncture, cardiopulmonary resuscitation, nebulization, passage of nasogastric tube, incision and drainage of abscesses.
• Updated and maintained accurate patient records.
• Ensured the supervision of junior team members and nurses to ensure patients' safety.

• Completed Internship rotation through medicine, surgery, pediatrics, obstetrics and gynecology.
