Delivery of holistic evidence based nursing care within the community setting for patients in their own homes, prioritising and managing caseloads on daily basis, responding to emergencies, and management of diverse acute and chronic conditions.
Patient visits, medication management, catheter care, end of life care, pleural and ascitic drainage, diabetes care and management, simple and complex wound management.
General observations – EWS charts, Blood glucose and blood pressure monitoring; Personal care, Supporting patients with feeding, ECG, Recording patient’s information both on EPR and paper.
Performing all roles in the dispensing process, responding to symptoms, minor ailments and counter prescribing, health promotion, diabetes clinics, organising smoking cessation workshops, interacting with patients and other healthcare professionals on daily basis.
