In my role as Relevant Person Representative (RPR) under the Mental Capacity Act 2005, I advocate on behalf of the expect by experience to ensure that the care and restrictions are necessary, proportionate, and in line with their needs. Also, my role requires empowering them to have a voice in their care decisions, even when their communication abilities are limited.

• Ensuring the Deprivation of Liberty Safeguards (DoLS) are necessary and acting to stop unjustified restrictions.
  
• Visiting the client regularly to understand their views, even if they cannot explicitly communicate them, and acting on their behalf.
  
• Requesting reviews of the authorisation and supporting individuals to appeal to the Court of Protection if they object to their care or placement.
  
• Ensuring the care setting adheres to the conditions of the authorisation.
  
• Acting as an independent voice separate from the care provider and local authority.

• Working in the integrated discharge team (IDT).
• liaising with the ward doctors and nurses to facilitate patient discharge destinations
• Arranging the care package and the restart of care.
• Arranging transport for service user to their discharge destinations.
• Visiting service users in the hospital ward to gather information.
• Completing a 28-day review in the care homes and the community
• Co-work with the occupational therapy for an assessment visit to the service user.
• Completing duty cases and referring clients to the appropriate services in the community that meet their needs.

• Supporting service users who have mental illness and older adults in a residential environment
• Conducted well-being meetings with service users according to their support plan, looking at progress and areas of need and drawing up action points on how to address the need

• Attending training on drug and alcohol abuse, which helped me to improve my knowledge on this topic
