Working within the Single Point of Access team based in South Block

Undertaking of assessments, both planned and unplanned, at various locations across the Island.

Answering of crisis phones and responding appropriatly to people who are in crisis; signposting as required.

Assessing and reviewing referrals make to the service.

Liaising with other parts of the service to ensure smooth operation.

Ensure clients visit other professionals for medication or comsultant reviews etc.

Laise with local authourity regarding housing and S117 aftercare needs.

Worked as a staff nurse in the Acute Admissions ward of a meduim secure unit.

Ensuring risk assessments and care plans were correct, relevent and up to date.

Completion of reports for both consultants and courts.

Support transport to courts, other units, or back to prison.

Enure complex medication regemes are administered correctly and documented in a timely manner.

Building a rapport with people needing to use the service, some of whom were very distressed and mentally unwell.

Working on an OPMH ward as a support worker.

Supporting the qualified staff nurses with tasks.

Supporting the day to day running of the ward, including supporting patients with their ADLs.
