Western Hospital OPMH is a service that provides assessments, diagnosis, treatments and support for those over the age of 65

My responsibilities:

• Provide explanations of treatment options for both patients and carers.
• Provide initial assessments, memory assessments and risk assessments.
• Provide accurate documentation of contact with patients.
• Complete shifts alongside university-based learning.
• Improve patient comfort by providing personalised care and attention.
• Attend home visits.
• Assess patients' mental state and potential signs of deterioration.

Team Overview:

Hamtun Ward is a psychiatric intensive care unit (PICU) for adult males. This service provides 24-hour care to those detained under the mental health act or in an informal stay

My responsibilities:

• Nurse in charge role.
• Perform physical assessment by measuring blood pressure, pulse rate and oxygen level.
• Medication Management by performing medication rounds accurately and safely.
• Assist patients with daily activities such as eating, bathing whilst promoting independence where possible.
• Build positive rapport with patients from various backgrounds.
• Work with the clinical team to carry out prescribed treatments addressing individual patient needs through attending ward rounds.
• Assess for the need of further seclusion via attending seclusion reviews.

Team Overview:

Royal Hampshire's Psychiatric Liaison service is there to provide Mental health assessments to those who have come to A&E and are struggling with their mental health

My responsibilities:

• Provide signposting to other services for ongoing support.
• Liaise with A&E for safe discharge, incorporating the patients' physical, mental and social needs.
• Communicate with patients and family members and de-escalate situations when necessary.
• Provide patient assessments with minimal supervision.
• Attend night shifts, as well as a mix of longer and shorter day shifts.
• Assess mental state.
• Document patient contact accurately.

Team Overview:

April House provides a day service for patients, which involves therapy group sessions and supervised meals

My responsibilities:

• Supervising meal times.
• Building therapeutic relationships.
• Carrying out initial assessments.
• Advised on dietary, nutrition and illness prevention.
• To regualry take the vital signof the patients to ensured stability or identify potential emergencies promptly.
• Provided emotional support, resulting in improved patient morale.

Melbury Lodge is a Male Acute ward, providing 24-hour care for those detained under the mental health act or as an informal stay

My responsibilities:

• Building therapeutic relationships.
• Accurately recording documentation.
• Carrying out mental state examinations.
• Improve patient comfort by providing personalised care and attention.
• Handling patients' hygiene needs in support of personal dignity and optimum wellness.
• Ensure a safe environment with regular cleaning.
• Deal with emergencies with calmness and composure.

Team Overview:

Cannon House provides a service for early intervention psychosis, providing a depot clinic and therapy

My responsibilities:

• Provide care for patients via reviews during home visits.
• Assist clinicians with medications in the clinic.
• Attend Psychology appointments.
• Build rapport with patients.
• Attend appointments with doctors.

Team Overview:

College Keep CMHT service provided appointments and telephone calls to ensure the safety and assess patients

My responsibilities:

• Shadow patients' initial assessments
• Join and shadow the physical health clinic
• Attend psychology sessions
• Build a positive rapport with patients from various backgrounds.
• To use techniques and devices to overcome language barriers.

My responsibilities:

• Handle cash transactions accurately.
• Communicate effectively with colleagues.
• Organise stock shelves neatly to improve the appearance of the shop floor.
• Manage time efficiently during peak hours to limit customer waiting periods.
• Advise customers on availability, pricing and location of products.
• Listen actively to customer concerns.
