Ward A524 - Care of the elderly (02/01/2023 - 19/02/2023)

• Provided person-centred care to Older Adults experiencing a range of conditions such as dementia, Parkinson's disease, Chronic diseases such as Diabetes, Heart failure, COPD and End of Life patients.
• Handled patients' hygiene needs in support of personal dignity and optimum wellness.
• Assisted in mobilising and transferring patients safely using appropriate equipment.
• Assisted physically challenged individuals in daily activities such as bathing eating mobilising leading enhanced quality of life.
• Administered prescribed medication to ensure timely treatment, especially with timed Parkinson’s medication.
• Offered friendly, efficient customer service and handled challenging situations with ease.

Online Placement

Clinical Training (19/02/2024 - 14/04/2024)

• Research around the rationale behind and functions of the NHS Health Check programme by reading the best practice guidelines
• Completed an NHS e-learning course on NHS Health Checks.
• Completed an NHS Health Check for a virtual patient on:

- QRISK® 3 Score

- GPPAQ for physical activity

- FAST screening for alcohol consumption.

- QDiabetes screening

- Mental Health Assessment

• Completed a face-to-face consultation with a patient with clear documentation of all results
• Evaluated the results of the NHS Health Check and identification of any risk factors for the patient
• Performed a video consultation to communicate the NHS Health Check results to a patient

Placement History

Endoscopy Unit (24/04/2023 - 18/06/2023)

• Gained experience through observing endoscopic procedures including -Gastroscopy,PEG placement, flexible sigmoidoscopy, colonoscopy, bronchoscopy, EBUS, enteroscopy, capsule endoscopy, EUS, ESD
• Gained experience within caring for patients having an endoscopic procedure, incorporating assessment and admission, care of the patient having a procedure, recovery and to discharge or transfer back to ward
• Assisted doctors alongside a registered nurse during procedures, ensuring success.

Acute Stroke Unit 34B (23/10/2023 - 17/12/2023)

• Strengthened my communication skills by requiring clear, compassionate interactions with patients who had speech or cognitive difficulties. Teaching me to adapt my communication style to support patients, families, and the wider multidisciplinary team.
• Responded to and interpreted neurological observations and assessments.

Surgical Trauma and Orthopaedic Ward 25A (09/09/2024 - 17/11/2024)

• Facilitated the recovery of patients through constant monitoring and evaluation of condition.
• During this placement I have gained a lot of knowledge through working alongside experienced nurses.
• I gained confidence within my medication rounds, wound care, the role of the nurse in charge, documentation and mobilising patients that required specialised equipment or care.
• High NEWS scores, raising concerns to doctors is something I had the opportunity to do on many occasions.
• Post operative observations : understanding the monitoring frequency and protocol post op, recognising signs of deterioration and carrying out an A-E assessment.
• Addressed patient pain levels, helping to promote healing by minimising discomfort.

Management placement : Surgical Urology 33B (17/02/2025 - 01/06/2025)

• Conducted informative handovers at shift end through concise and accurate reporting and communication.
• Dressed surgical wounds, monitored healing and replaced dressings using ASEPTIC techniques to prevent infections.
