`I worked as a care assessor in a community Hospital Setting. I was responsible for completing assessments for individuals who were ready to be discharged from hospital. I would complete their assessment and funding of the level of support I was requesting. I would also then complete a 4-6 week review to check whether they needed the same level of support or increase/decrease in care. I worked in line with the Fair access to care eligibility criteria. I worked in a multidisciplinary team working with many other professionals. My work also including safeguarding and mental capacity assessments.

I worked for careline, answering emergency calls, installing carelines in individuals homes.
