As the acting team manager and team manager my main responsibilities are: Continuing to develop the service whilst ensuring smooth day to day running of the team, supervising band 6,5 and 4 staff and completing supervisions and appraisals. Completing staff specific tasks such as absence, probation, annual leave requests, mileage and lone working procedures. I prioritise and allocate referrals received, ensuring that initial assessments and contact targets are achieved. I raise safeguarding incidents and IR1 for incident management as well as support members of the team to raise safeguarding incidents. I ensure that clinical governance and auditing is completed for the team and that information is fed back up to the service manager and the clinical commissioning group. I audit and collate information from the team's statistics for a monthly report that I complete for presentation to the clinical commissioning group on a monthly basis. I liaise with multiple professionals and provide clinical visits to service users with complex needs within the community and inpatient settings to provide comprehensive assessment/care planning/interventions and monitoring following a proactive or crisis intervention route. I attend blue light meetings, community treatment reviews , Care Programme Approach (CPA) and psychiatry reviews. I work closely with the consultant psychiatrists/General Practitioners (GP) to review and monitor the physical and mental health well being of each service user as well as monitor medication changes for the service users within my care. As part of my role I also provide training in positive behavioural management to Avon and Wiltshire Partnership staff and bespoke training to the service users carers/family/care providers tailored specifically to the service user and their presenting need. I have started to undertake investigations as part of my role and to write reports for the investigations completed. I handle compliments and complaints that arise from service users, families, carers, professionals and care providers with a sensitive manner and timely fashion.
Establishing a new pilot scheme to work with people with learning disabilities, mental health and/or challenging behaviour as a senior member of staff.Developing pathways for the service and a crisis care pathway with multiple professionals. Liaising with multiple professionals and outside agencies to ensure that the holistic care needs of the service users are met. Ensuring assessments and reports were completed in a timely fashion and referrals to appropriate professionals were made following assessment. Supervision and appraisals of the band 5 and band 4 staff.Reporting Safeguarding incidents and raising IR1 incident forms.Ensuring that clinical governance and auditing for the team was completed in a timely manner and reported on to the service manager. Working closely with the co-located and integrated health and social care team, psychiatry, GP, primary and secondary care sectors to ensure that reasonable adjustments were made for service users with a learning disability to access mainstream services.
I was working in an integrated health and social care team for people with a learning disability. I managed my own caseload of service users with a learning disability, mental health and/or challenging behaviour's completed assessments such as; initial, contact and risk assessments, care plans, capacity assessments and CPA crisis and contingency plans. I liaised with primary care and other mainstream services to ensure that the service users within my care received the most beneficial and holistic care and reasonable adjustments were made when required. I worked closely with the service user/family/carers/advocates and the MDT to ensure continuity of care.I prepared cases to present at funding panels when additional funding needed to be sourced to keep the service user at home with additional support from their care provider facilitated training on a document called "What's the plan" which is an easy read plan for people with a learning disability and mental health as part of the CPA training. I also facilitated positive behavioural management and epilepsy training.I was the care coordinator for service users on the CPA care pathway and I completed positive behavioural management plans, CPA assessments/reviews, risk assessments and crisis and contingency plans.
For a 6 month part of this employments I spent: (Health Facilitation Nursing) One day a week I spent working with service users with physical health issues. I liaised with GP surgeries, hospital liaison nurses, completed health action plans, hospital passports and dementia triage. I also held nurse led epilepsy clinics and completed epilepsy management plans and emergency management plans.
(Continuing healthcare CHC Nursing) One day a week I spent completing CHC assessments and reviews as well as capacity assessments and arranging best interest meetings.
The dietetics team covered Bristol, South Gloucestershire and North Somerset . As a dietetics assistant I would liaise and provide home visits with service users/families and carers to support them with the care plans that had been put into place. I supported the dietetics service with training primary care/GP practice staff to understand the holistic and dietary needs of people with a learning disability. I was seconded to the University of the West of England (UWE) in 2009 and qualified as a learning disability nurse in 2012. Whilst I was waiting for my registration/pin to come through I worked as a dietetics assistant.
