I was working in an integrated health and social care team for people with a learning disability. I managed my own caseload of service users with a learning disability, mental health and/or challenging behaviour's completed assessments such as; initial, contact and risk assessments, care plans, capacity assessments and CPA crisis and contingency plans. I liaised with primary care and other mainstream services to ensure that the service users within my care received the most beneficial and holistic care and reasonable adjustments were made when required. I worked closely with the service user/family/carers/advocates and the MDT to ensure continuity of care.I prepared cases to present at funding panels when additional funding needed to be sourced to keep the service user at home with additional support from their care provider facilitated training on a document called "What's the plan" which is an easy read plan for people with a learning disability and mental health as part of the CPA training. I also facilitated positive behavioural management and epilepsy training.I was the care coordinator for service users on the CPA care pathway and I completed positive behavioural management plans, CPA assessments/reviews, risk assessments and crisis and contingency plans.

For a 6 month part of this employments I spent: (Health Facilitation Nursing) One day a week I spent working with service users with physical health issues. I liaised with GP surgeries, hospital liaison nurses, completed health action plans, hospital passports and dementia triage. I also held nurse led epilepsy clinics and completed epilepsy management plans and emergency management plans.

(Continuing healthcare CHC Nursing) One day a week I spent completing CHC assessments and reviews as well as capacity assessments and arranging best interest meetings.

I worked with all the GP surgeries that had signed up to the direct enhanced service annual health check scheme. This involved liaising with with the link from each surgery and checking the registers. I audited the information collated at the surgeries to identify service users who had not had an annual health check and liaised with the GP's to identify if a referral would be needed to the CLDT for support to access this service. I produced spreadsheets each month and presented these to the clinical commissioning team. I would receive referrals and support the service users to access their surgeries with reasonable adjustments to have their annual health check and follow up any outcomes.
