My background as a Diabetes Specialist Nurse includes experience in delivering high-quality, patient-centred diabetes care within Secondary care and more recently, the Primary care environment. I am competent and confident in my ability to support both patients and healthcare professionals through structured education, training, and evidence-based interventions to facilitate individuals with diabetes to have the skill set necessary for self-management.
Role responsibilities include but are not limited to; insulin and OHA initiation and titration, improving clinical outcomes, recent experience with supporting type 1 individuals on pump therapy (including Omnipod, Ypsomed, T-Slim and Medtronic systems), reviewing Glooko and Carelink and commencing on appropriate CGM options available within the UK market. I am invested in the developments and innovation in diabetes technology, working towards improving the quality of life for people living with diabetes and reducing mental load.
Prior to this, I worked within an acute hospital trust which enabled me the opportunity to become upskilled in managing critically unwell patients, ranging from a short stay medical unit to a Covid HDU. I pride myself on my commitment to continuous self-development and the ability to adapt quickly in response to an ever changing environment and the challenges that this can present.
· Co-ordinate ward area and day to day provision of nursing services working within wider MDT, to optimise clinical outcomes and support patient flow
