Personalinformation - Responsibilities

  • Coordinating the care and assessment under the Care Programme Approach of a designated caseload;
  • Responsible for the admission, case conference/reviews and discharge in collaboration with members of the multi-disciplinary team;

  • Liaison with essential stakeholders such as the social service, housing, police, primary care plus, families, probation service, solicitors, and charity organisations.
  • Sustaining holistic interventions and continual risk assessment, collaborative crisis, contingent plan & making referrals to the AMHP team when patients are acutely unwell.

Clinicalresponsibilities

  • Maintain high standards of care, working with service users to ensure their individuality and dignity are maintained while promoting independence.
  • Ensure that each patient in his/her care receives an individual assessment and regularly updated care plan which is formulated with a service user.
  • Ensure that information regarding the service user's needs and progress are communicated promptly and accurately to other people involved in their care, having due regard for confidentiality.
  • Use a Behavioural Family Therapy approach to care for service users where appropriate.
  • Ensure that the requirements of the Mental Health Act 1983 and Mental Capacity Act 2005 are met and that service user's rights under the Acts are upheld.
  • Facilitate open and harmonious communication amongst staff within the team and other teams and agencies to promote multidisciplinary approaches to care.
  • Work as CPA Care Co-ordinator to service users where required
  • Provide information and support for service users and families
  • Liaise closely with other statutory and voluntary agencies where required.
