
I work as a general & acute medicine registrar since 18/9/2025 .As a medical registrar I'm responsible for the
assessment, diagnosis, and management of acutely unwell medical patients. My role includes clinical leadership
during ward rounds, supervision of junior doctors, and active participation in departmental governance and service
delivery.
My Clinical Responsibilities:
1- Acute Medical Take
* Lead the acute medical take, ensuring timely assessment and management of newly admitted patients.
* Perform senior clinical reviews of acutely unwell patients in areas such as:
* Emergency Department (ED)
* Acute Medical Unit (AMU)
* Same Day Emergency Care (SDEC)
* Prioritise patients based on clinical acuity.
* Initiate investigations and management plans in line with best practice.
* Escalate care appropriately (e.g. ICU referral when needed).
* Ensure accurate and timely documentation
2-Management of Acutely Unwell Patients
* Provide immediate assessment and stabilization of deteriorating patients.
* Lead resuscitation efforts where required.
* Apply clinical frameworks such as:
* ABCDE assessment
* Sepsis pathways
* NEWS2 escalation protocols
* Work collaboratively with multidisciplinary teams (MDT), including nursing staff, allied health professionals, and
critical care teams.
3- Leadership and Ward Rounds
3.1 Leading Ward Rounds
* Conduct daily consultant-led or registrar-led ward rounds.
* Coordinate and lead a team of junior doctors (FY1, FY2, IMTs).
* Ensure:
* Clear management plans are made
* Tasks are appropriately delegated
* Patient safety is prioritized
* Promote efficient patient flow and discharge planning.
3.2 Supervision and Teaching
* Provide direct clinical supervision to junior doctors.
* Offer bedside teaching and case-based discussions.
* Support juniors in:
* Clinical decision-making
* Procedural skills
* Professional development
* Ensure safe prescribing and clinical practice
4-Communication Responsibilities
* Communicate effectively with:
* Patients and families regarding diagnosis and management
* Senior clinicians and consultants
* Multidisciplinary teams
* Provide clear handovers using structured tools (e.g. SBAR).
* Maintain high-quality clinical documentation.
5-On-Call Duties
* Act as the senior decision-maker during on-call shifts.
* Provide advice to junior doctors across wards.
* Review deteriorating patients hospital-wide when required.
* Coordinate with consultants for escalation.
6-Weekly Departmental Meetings
* Attend and actively participate in weekly departmental meetings.
* Contribute to:
* Case discussions
* Present cases from the acute take
• Inpatients: My responsibility is to admit patients with various medical conditions including (Cardiology conditions), start the management and carry out the consultant recommendations including all steps of taking history and physical examination, communicate with the patient and the family, order necessary tests, discuss the case with the consultant and follow up the patient progress. As far as procedures, I started learning the theoretical aspect of procedures and participated in preparing and consenting patients for that. During that period, I passed the pertinent exams and became an MRCP (Member of the Royal College of Physicians). I was subjected for annual reviews by the supervising consultants and the department chief, and he judged my competencies.
• CCU: Managing patients admitted in CCU with different medical & cardiology conditions.
• Outpatient: High experience in conducting outpatient and outreach clinics. I have refined my consultation skills to ensure patient satisfaction whilst recognizing the importance of running my clinics to time. I enjoy reflecting on selected patients that I have seen with senior Input, to enhance my learning and understanding.
• Coronary Intervention: I was given the chance to attend and observe numerous coronary interventions. Dr Erfan Hospital is a well-known center for performing coronary intervention nationally.
